PLDD联合针灸及杜仲腰痛丸治疗腰椎间盘突出症临床观察

摘　　要：目的:观察分析经皮激光汽化减压术(Percutaneous laser discdecompression,PLDD)联合针灸及杜仲腰痛丸治疗腰椎间盘突出症(Lumbar dise herniation,LDH)的临床疗效。方法:回顾性分析2013年5月-2016年6月在甘肃省中医院脊柱骨科采用PLDD联合针灸及杜仲腰痛丸治疗的117例LDH患者的临床资料,按不同疗法分为3组:A组(PLDD+针灸组)患者37例;B组(PLDD+中药组)患者41例;C组(PLDD+针灸+中药组)患者39例。记录各组不同时间点VAS疼痛评分,评价术后随访2个月的临床疗效,分析各治疗手段的有效性,总结治疗经验。结果:治疗后1周、2周、1个月、6周和2个月,C组VAS评分明显低于A、B组,且C组疼痛评分的减轻程度显著高于A、B组,差异具有统计学意义(P<0.05);术后随访2个月,A、B、C3组有效率无明显统计学意义(P>0.05),优良率分别为67.6%、65.9%、76.9%,C组明显高于A、B两组,差异具有统计学意义(P<0.05)。结论:运用PLDD联合针灸及杜仲腰痛丸治疗LDH疗效显著,值得临床推广运用。Objective:To observe and analyze the clinical efficacy of percutaneous lasers disk deoompression(PLDD)combined with acupuncture and Duzhong Yaotong Pill in the treatment of lumbar disc herniation(LDH). Methods:Retrospective analysised the clinical data of 117 cases LDH patients,which accepted PLDD combined with acupuncture and Duzhong Yaotong Pill in the spinal surgery of Gansu provincial hospital of TCM from May. 2013 to Jun. 2016,and according to the different combination treatments were categorized groups:Group A(PLDD+acupuncture)37 cases;Group B(PLDD+traditional Chinese medicine)41 cases;Group C(PLDD+acupuncture+traditional Chinese medicine)39 cases,the VAS scores at different time points and the clinical efficacy after 2 mouths were recorded and evaluated,and the effectiveness of each treatment method was analyzed and summarized experience of treatment. Results:The VAS pain scores in group C were significantly different from those in group A and B at 1 week,2 weeks,1 month,6 weeks and 2 months after treatment in all patients,and C group had significantly higher degree of relieve pain score group A and B,it had statistically significant difference(P<0.05);the therapeutic evaluationtion of the each group showed that the effective rate of group A,B and C was not statistical significance in 2 months after treatment(P>0.05),while the excellent rate was 67.6%,65.9% and 76.9% respectively,and the group C was significantly higher than that of group A and B,and the difference was statistically significant(P<0.05). Conclusion:The use of PLDD combined with acupuncture and Duzhong Yaotong Pill is effective in treating LDH and It is worthy of clinical application.
