手术方式的改进在加快治疗浆细胞性乳腺炎中的研究

摘　　要：目的探讨分析手术方式的改进对浆细胞性乳腺炎治疗效果的影响。方法选取2010年1月～2016年8月本院乳腺外科收治的105例浆细胞性乳腺炎女性患者作为研究对象,按照治疗方法的不同将患者分为实验组和对照组,又根据炎症的浸润程度将实验组分为实验1组和实验2组,实验1组为单纯炎性肿块期,病变发展为脓肿形成并伴有皮肤破溃者为实验2组。实验组通过运用改进手术方式方法进行治疗,实验1组一期行完整炎性肿块切除术,放置负压引流管行一期缝合皮肤;实验2组一期切除炎性肿块,二期放置负压引流并整形处理。对照1组采用传统切开换药;对照2组采用中医中药治疗。比较治疗前后不同组患者的临床疗效、住院时间、美容效果、患者主观疼痛情况。结果实验组通过改进手术方式治疗后乳房外形程度的改变小于对照组,在病房的住院时间短于对照组,两组比较差异有统计学意义(P<0.05)。实验组疼痛缓解情况比对照组更优,两组比较差异有统计学意义(P<0.01)。实验组与对照组性别及年龄方面无统计学差异(P>0.05)。结论应用改进手术方式治疗浆细胞性乳腺炎可明显缩短患者的住院时间,减少患者换药痛苦,并且术后乳腺外形影响小,美容效果好,患者满意度高。Objective To explore and analyse the effect of rapid surgical treatment of plasma cell mastitis.Methods A total of 105 patients with plasma cell mastitis retraction from January 2010 to August 2016 in breast surgery of our hospital were selected.According to thetreatment methods,the patients were divided into experiment groups and control groups.The experiment groups were divided into two group based on the degree of infiltration of inflammation,experiment group 1 with inflammatory mass lesion development period,and experiment group 2 with abscess formation and ulceration of the skin.The experiment group 1 was treated with a complete inflammatory mass removed,the negative pressure drainage tube was placed in the first phase of skin suture.The experiment group 2 was treated with inflammatory mass removed in the first phase,and negative pressure drainage was placed and plastic treatment in the second stage.The control group 1 was treated with traditional incision and dressing change,and control group 2 was treated with traditional Chinese medicine.The clinical efficacy,hospitalization time,cosmetic effect and pain reliefbefore and after treatment between the different groups were compared.Results In the experimental group,the change of the shape of the breast after rapid surgery was less than that in the control group,the hospitalization time was shorter than the control group,the difference was statistically significant between the two groups(P<0.05).Compared with the control group,the pain relief in the experimental group was better than that in the control group,and the difference was statistically significant between the two groups(P<0.01) in the experimental group and the control group with no significant difference in gender and age(P>0.05).Conclusion The application of improvement of surgical procedure in plasma cell mastitis can shorten the hospitalization time,reduce patient dressing pain,and improved satisfaction of postoperative breast shape,good cosmetic effect.
