摘 要:目的探讨以可调式单边外固定器固定,局部注射成骨诱导后的骨髓间充质干细胞促进萎缩性骨不连修复的效果。方法取36只雄性Lewis大鼠随机分为3组,A组:对照组,B组:骨不连组,C组:实验组。于右侧股骨骨干处用骨锯截断股骨。B、C组用电刀将断端2 mm以内的骨膜破坏并冲出骨髓。B组于截骨区注射纤维蛋白胶; C组注射含有2×10~6/mL骨髓间充质干细胞的纤维蛋白胶。定期摄X线片,于术后8周取材,分别行大体、组织学、力学检测观察愈合情况。结果在8周时,外固定器均保持正常位置,整个实验期间,所有动物股骨对位良好。所有检测结果证实在第8周时,A组骨断端已经自然愈合,髓腔再通; B组截骨区无骨性连接,截骨端未愈合,截骨间隙略有增宽; C组截骨端有连续性骨痂通过,桥接截骨区。结论经成骨诱导后的骨髓间充质干细胞复合纤维蛋白胶注射到萎缩性骨不连处可以促进其修复,促进骨愈合的是骨髓间充质干细胞而不是纤维蛋白胶。Objective To evaluate the efficacy of locally injected mesenchymal stem cells promoting the repair of bone atrophic nonunion,which is stabilized with an adaptable external fixation system. Methods Thirty-six male Lewis rats were divided randomly into three groups:group A: control group;group B: nonunion group;group C: experimental group. The rats underwent a right femoral osteotomy,which was stabilized with the external fixator. Except for the control group,in all animals a bone atrophic nonunion was induced by cautering the periosteum and removing the bone marrow. Fibrin glue was injected into the osteotomy gap in group B;2× 10^6/mL mesenchymal stem cells were injected into the osteotomy gap in group C. Results Correct positioning of the external fixator was confirmed by postoperative X-rays. Throughout the entire examination period,all femurs retained their positioning. After 8 weeks,the group C showed complete bony bridging,whilst a bone atrophic nonunion was detected in the group B.All outcome measures showed better healing of the group A compared with the group C. Conclusion Locally injected mesenchymal stem cells could promote the repair of bone atrophic nonunion,which is mesenchymal stem cells instead of fibrin glue.
