摘  要:炎症性肠病(IBD)是一种与宿主遗传因素、免疫系统失调和环境因素紧密相关的非特异性肠道炎症性疾病,可累及回肠、直肠、结肠等器官,包括溃疡性结肠炎(UC)和克罗恩病(CD)。消化内镜在UC和CD的诊断,以及在对抗炎治疗、黏膜愈合程度、炎性肠病相关癌症的密切监测中发挥着重要作用。与传统的白光内镜检查相比,细胞内镜系统(ECS)能更精确地表征病变,可对活体组织进行类似于显微镜下的观察,确定病变的部位和性质,增加内镜下诊断的准确性,减少诊断步骤,以期能为IBD诊断和临床监测开辟新的途径。本文对ECS在IBD诊治中的应用进展进行总结。Inflammatory bowel disease(IBD), including ulcerative colitis(UC) and Crohn’s disease(CD), is an idiopathic intestinal inflammatory disease which is closely connected with host genetic factor, immune system disorder and environmental factors, would influence the ileum, rectum and colon as well. With the gradual increase of incidence, the endoscopy plays an important role in diagnosis of UC and CD and timely monitoring of anti-inflammatory treatment, mucosal healing and cancers related to inflammatory bowel diseases. Compared with traditional white light endoscopy, endocytoscopy, an advanced monitoring technique, could more precisely reflect lesion, predict mucosal inflammation, and carry out observation of living tissue cell structure similar to that under microscope, determine the location and nature of lesion, and thus increase precision of endoscopic diagnosis and reduce diagnosis steps, this could possibly pave a new way for IBD diagnosis and clinical monitoring. This paper summarizes recent advances in application of endocytoscopy in the diagnosis of IBD.
