摘　　要：目的:分析下腔静脉变异的超声表现,探讨超声检查对下腔静脉变异畸形诊断的临床价值。方法:回顾分析2008年1月日至2017年11月30日,在四川大学华西医院超声检查发现的12例下腔静脉变异畸形的声像图表现,以及该疾病并发的其他相关疾病的起因。结果:超声检查发现12例下腔静脉变异患者,其中8例(8/12)为左位下腔静脉,3(3/12)例为双下腔静脉、1例(1/12)肝段下腔静脉缺如,12例患者中1例双下腔静脉伴左侧下腔静脉"胡桃夹现象",1例左位下腔静脉伴"胡桃夹现象",1例左位下腔静脉伴左侧髂静脉血栓及1例左位下腔静脉伴双侧髂静脉血栓和双下肢静脉血栓。结论:发现异常下腔静脉的位置及走行对诊断心血管畸形、制定介入治疗及外科手术方案具有非常重要的意义。同时,发现该疾病的相关并发症,加深对该疾病的认识,尽早及时为临床提供可靠的影像学资料,提示患者下腔静脉变异畸形,使临床及时调整相关诊疗方案,提高治疗的准确度,避免意外损伤。Objective: To analyze ultrasonography image of the inferior vena cava malformation and discuss the clinical value of diagnosing inferior vena cava malformation by ultrasound. Methods: The ultrasonography image of 12 patients with inferior vena cava malformation and the cause of related complications were retrospectively analyzed in the West China Hospital of Sichuan University from January 1, 2008 to November 30, 2017. Results: Of the12 patients checked by ultrasound, 8(8/12)cases existed the left inferior vena cava,3(3/12)cases had double inferior vena cava,1(1/12)case had absence of inferior vena cava in the hepatic segment of inferior vena cava.In addition,the double inferior vena cava accompanied with the left inferior vena cava "nutcracker phenomenon" was found in 1 case, the left inferior vena cava with "nutcracker phenomenon" existed in 1 case, the left inferior vena cava with left iliac vein thrombosis was found in 1 case, and the left inferior vena cava with double iliac vein thrombosis and both lower extremities vein thrombosis was found in 1 case. Conclusions: It has significant meaning to diagnose cardiovascular malformation, for drafting interventional therapy and doing surgical operation by finding the abnormal location and walking line of the inferior vena cava, deeply understanding of the disease, providing reliable ultrasonic imaging data as early as possible. Moreover, the discovery of related complications can lead to adjust the clinical diagnosis scheme timely, improve the accuracy of treatment and avoid accidental injury.
