活血降糖饮对2型糖尿病患者饮食偏好的影响及分子机制研究

摘　　要：目的探讨活血降糖饮对2型糖尿病患者高糖高脂饮食偏好的影响以及对血浆可溶性CD36的调控作用。方法选取我院住院及门诊患者共40例,分为治疗组和对照组,各20例,均给予磷酸西格列汀口服治疗,治疗组加用活血降糖饮。治疗前后分别计算证候积分,检测空腹血糖(FPG),餐后2小时血糖(2h-PG),空腹胰岛素(FIns),餐后2小时胰岛素(2h-Ins),胰岛素抵抗指数(HOMA-IR),游离脂肪酸(FFA),s CD36,以及对4种不同糖脂含量的饮品的偏好得分。结果经治疗,两组患者证候积分、FPG、2h-PG、Fins、2h-Ins、HOMA-IR、FFA、s CD36以及对高糖高脂饮食的偏好得分均明显改善(P <0. 05,P <0. 01)。经治疗后,治疗组证候积分、2h-PG、Fins、2h-Ins、HOMA-IR、FFA、s CD36以及对高糖高脂饮食的偏好得分均较对照组改善更加明显,差异有统计学意义(P <0. 05,P <0. 01)。结论活血降糖饮具有良好的降糖调脂改善胰岛素抵抗的作用,同时能够调节血浆s CD36含量,并能够改善2型糖尿病患者对高糖高脂饮食的偏好。Objective: To explore the effect of Huoxue Jiangtang Decoction on high glucose and high fat diet preference in patients with type 2 diabetes mellitus and its regulation on plasma soluble CD36. Method: A total of 40 inpatients and outpatients in our hospital were selected and divided into treatment group and control group,with 20 cases in each.All patients were given Sigliptin Phosphate Oral Treatment,and the treatment group was given Huoxue Jiangtang Decoction in addition. Syndrome scores were calculated,fasting blood glucose( FPG),2-hour postprandial blood glucose( 2 h-PG),fasting insulin( FIns),2-hour postprandial insulin( 2 h-Ins),insulin resistance index( HOMA-IR),free fatty acid( FFA),s CD36 and preference scores for four drinks with different glycolipid contents were measured before and after treatment. Result: After treatment,the scores of syndromes,FPG,2 h-PG,Fins,2 h-Ins,HOMA-IR,FFA,s CD36 and preference for high-sugar and high-fat diet were significantly improved in both groups( P<0.05,P < 0.01). After treatment,the scores of syndromes,2 h-PG,Fins,2 h-Ins,HOMA-IR,FFA,s CD36 and preference for high-sugar and high-fat diet in the treatment group improved more significantly than those in the control group,the difference was statistically significant( P<0.05,P<0.01). Conclusion: Huoxue Jiangtang Decoction has a good effect of lowering blood sugar and lipid,improving insulin resistance,regulating plasma s CD36 content,and improving the preference of type 2 diabetes mellitus for high-sugar and high-fat diet.
