不同卵巢反应中LHR表达与卵泡液IGF-I、IGFBP-1表达及IVF结局的相关研究

摘　　要：目的了解卵泡液颗粒细胞黄体生成素受体(LHR)表达水平与卵巢反应性、卵泡液IGF-I、胰岛素生长因子结合蛋白-1(IGFBP-1)表达以及IVF结局之间的关系。方法选取因输卵管因素行IVF治疗的不孕妇女109例患者。患者全部使用标准长方案,根据取卵日获卵数分为3组:获卵数≤3个的患者为低反应组,3个<获卵数<15个的患者为正常反应组,获卵数≥15个的患者为高反应组。收集患者卵泡液基础雌二醇(E2)、IGF-1、IGFBP-1水平、获卵数、胚胎数、优胚数以及检测胚胎移植后的血HCG水平和B超结果等病历资料。结果 3组患者间卵巢颗粒细胞LHR蛋白、LHR mRNA表达量差异无统计学意义(P> 0. 05)。颗粒细胞LHR、LHR mRNA表达量与卵泡液IGF-1、IGFBP-1表达、卵泡数及获卵数、受精率、卵裂率、优胚率和妊娠率差异无统计学意义(P> 0. 05)。结论控制性卵巢刺激后,不同卵巢反应性患者,颗粒细胞上LHR表达水平没有差异性,不能作为判断反应性的指标。颗粒细胞LHR的表达与IGF-I、IGFBP-1表达、受精率、卵裂率、优胚率和妊娠率均无相关性。Objective To study the relationship between expression of LHR in follicular granulose cells and expression of IGF-1,IGFBP-1 in follicular fluid,ovarian response,IVF outcomes. Methods 109 infertile women of tubal factor undergoing IVF treatment in our reproductive center were chosen to study from. All the patients undergone IVF conducted with standard long protocol. The patients were divided into three groups according the number of oocyte obtained,less than 3 is poor response,more than 15 is hyperresponse,between 3 and 15 is normal response. Studying data including of E2,IGF-1,IGFBP-1 in follicular fluid,the number of oocyte obtained,embryo number,good embryo number,serum HCG level and B-utrasound. Results The level of LHR protein and LHR mRNA in follicular granulose cells were no significant difference between the three groups. There is no correlation between the expression of LHR in follicular granulose cells and follicular,number of retrieted oocytes,fertilization rate,cleavage rate,good embryo rate,pregnancy rate. Conclusion There was no difference in level of LHR between the different ovarian response groups in controlled ovarian stimulation,can not be used as a indicator of ovarian response. There was no relationship between the expression of LHR and level of IGF-1,IGFBP-1,fertilization rate,cleavage rate,good embryo rate,pregnancy rate.
