摘  要:目的探究糖基化白蛋白和糖基化血红蛋白水平与糖尿病患者冠心病狭窄程度的相关性及其临床意义。方法收集接受冠状动脉造影检查的糖尿病患者400例。根据冠脉病变程度,将所有受试者分成重度、中度和轻度3组。根据冠脉造影结果,将受试者分成冠脉正常组和冠脉病变组,计算相关系数,进行受试者工作特征曲线(ROC曲线)分析。结果所选受试者中,男性212例,女性188例,平均年龄(63. 76±10. 25)岁;平均Hb A1c(7. 69±1. 63) mmol/L,平均GA含量(14. 93±3. 52)%;重度、中度和轻度3组的HbA_(1c)、GA和LDL差异有统计学意义(P <0. 05);冠脉正常组和冠脉病变组间年龄、HbA_(1c)、GA、LDL、HDL、TC和Gensini积分相比,差异有统计学意义(P <0. 05);相关性分析表明LDL、HbA_(1c)和GA与冠状动脉损伤评分呈正相关,HbA_(1c)相关性最强(r=0. 537);研究结果显示,可以用HbA_(1c)、GA、LDL、HDL的ROC曲线下面积评价冠状动脉损伤程度,GA曲线下面积最大(0. 807)。结论 GA、HbA_(1c)水平与糖尿病患者冠心病狭窄程度具有相关性,检测其水平对糖尿病患者冠心病病情预防、诊断和治疗有重要意义。Objective To investigate the relationship between the levels of glycated albumin and glycated hemoglobin and the severity of coronary heart disease in diabetic patients and its clinical significance. Methods A total of 400 patients with diabetes who underwent coronary angiography were enrolled. The average age was 63. 76 ± 10. 25. All subjects were divided into three groups according to the severity of coronary lesions: severe,moderate,and mild. According to the results of coronary angiography,subjects were divided into normal coronary artery group and coronary artery disease group. Correlation analysis was performed using multi-factor correlation analysis,and correlation coefficient was calculated;and receiver operating characteristic curve( ROC curve) was analyzed. There was considered statistically significant( P < 0. 05). Results The selected subjects were 212 males and 188 females with an average age of( 63. 76 ± 10. 25) years;mean HbA1c( 7. 69 ± 1. 63) mmol/L,mean GA content( 14. 93 ± 3. 52%);in the severe,moderate and mild three groups,the differences of HbA1c,GA and LDL were different( P < 0. 05);the differences of age,HbA1c,GA,LDL,HDL,TC,SCR and Gensini scores in different groups were different( P < 0. 05);correlation analysis showed that LDL HbA1c and GA were positively correlated with coronary artery injury scores,and HbA1c was the strongest( r = 0. 537). The results showed that the extent of coronary artery injury could be evaluated by the area under the ROC curve of HbA1c,GA,LDL,and HDL. 0. 807. Conclusion The levels of early glycosylation products GA and HbA1c are correlated with the degree of coronary heart disease stenosis in diabetic patients,it indicating that the detection of glycosylation product is important for the prevention,diagnosis and treatment of coronary heart disease in diabetic patients.
