定喘汤联合氯雷他定治疗小儿哮喘合并变应性鼻炎疗效及对血清白细胞介素13、C反应蛋白和干扰素γ的影响

摘　　要：目的:观察定喘汤联合氯雷他定治疗小儿哮喘合并变应性鼻炎(AR)疗效及对血清白细胞介素13 (IL-13)、C反应蛋白(CRP)和干扰素γ(IFN-γ)的影响。方法:选取我院门诊诊治的哮喘合并AR患儿96例作为研究对象,按照随机数字表法分为研究组和对照组各48例,其中对照组给予氯雷他定治疗,观察组在对照组基础上联合定喘汤治疗。观察2组中医症候积分、临床疗效,检测患儿用力肺活量(FVC)、第一秒末用力呼气容积(FEV1)、峰值呼气流速(PEF)水平和IL-13、CRP、IFN-γ水平。结果:治疗后2组鼻塞喷嚏、气促胸满、咳嗽喘息、痰黏稠黄、咽干鼻痒积分显著低于治疗前(P<0. 05),且研究组显著低于对照组(P<0. 05);研究组总有效率91. 67%显著高于对照组72. 92%(P<0. 05);治疗后2组FVC、FEV1、PEF水平显著高于治疗前(P<0. 05),且研究组显著高于对照组(P<0. 05);治疗后2组IL-13、CRP显著低于治疗前(P<0. 05),且研究组显著低于对照组(P<0. 05),2组IFN-γ显著高于治疗前(P<0. 05),且研究组显著高于对照组(P<0. 05)。结论:定喘汤联合氯雷他定可有效改善哮喘合并AR患儿血清IL-13、CRP、IFN-γ水平和通气功能,提高临床疗效。Objective:To explore the influence of using Dingchuan Decoction combined with Loratadine in the treatment of children with asthma complicated with allergic rhinitis(AR) on curative effect,serum interleukin-13(IL-13),C-reactive protein(CRP) and interferon-γ(IFN-γ).Methods:96 cases of children with asthma complicated with AR treated in our hospital were selected as the study objects,and were divided into study group and controlled group according to random number table method,48 cases in each group.The controlled group were given Loratadine treatment,and the observation group were combined with Dingchuan Decoction on the basis of the controlled group.TCM symptom scores,clinical efficacy were observed,and forced vital capacity(FVC),forced expiratory volume(FEV1),peak expiratory flow(PEF) level,and IL-13,CRP,and IFN-γlevels were detected in the two groups.Results:After treatment,stuffy nose,sneezing,coughing,wheezing,shortness of breath,chest full of thick yellow sputum,throat and nasal itching scores were significantly lower than that before treatment(P<0.05),and those in the study group were significantly lower than those in the controlled group(P< 0.05).The total effective rate of the study group was 91.67%,significantly higher than that of 72.92% of the controlled group(P< 0.05).After treatment,FVC,FEV1 and PEF levels were significantly higher than those before treatment in the two groups(P<0.05),and those in the study group were significantly higher than those in the controlled group(P<0.05).After treatment,IL-13 and CRP levels were significantly lower than those before treatment in the two groups(P<0.05),and those in the study group were significantly lower than those in the controlled group(P<0.05).IFN-γ level was significantly higher than that before treatment(P<0.05),and that in the study group was significantly higher than that in the controlled group(P<0.05).Conclusion:Dingchuan Decoction combined with Loratadine can effectively improve serum IL-13,CRP,IFN-γ levels and ventilation of children with ast
