摘　　要：目的探讨后腹腔镜下输尿管切开取石术治疗输尿管镜钬激光或者气压弹道碎石失败后的输尿管L_5至骶髂关节水平结石的临床效果。方法回顾性分析2015年1月至2018年7月泗洪县分金亭医院泌尿外科8例行经尿道输尿管镜碎石术处理输尿管L_5至骶髂关节水平结石,由于进镜困难导致输尿管镜治疗失败后改行后腹腔镜下输尿管切开取石术患者的临床资料。结果 8例患者顺利完成后腹腔镜下输尿管切开取石手术,无副损伤,无并发症,结石清除率为100%。结论输尿管L_5至骶髂关节水平结石行输尿管镜碎石失败后中转经后腹腔镜下输尿管切开取石术治疗安全有效,值得在有条件的基层单位推广应用。Objective To explore the clinical effect of retroperitoneal laparoscopic ureterotomy for the treatment of ureteral stones at the horizontal level of L5 to sacroiliac joint after failure of ureteroscopy and holmium laser lithotripsy or pneumatic ballistic lithotripsy. Methods A retrospective analysis was performed on the clinical data of 8 patients who underwent ureteroscopic lithotripsy for the treatment of ureteral stones at the horizontal level of L5 to sacroiliac joint in the Department of Urology at the Sihong County Fenjinting Hospital from January 2015 to July 2018, but the treatment failed due to difficulty in entering the ureteroscope. Then these patients switched to receive retroperitoneal laparoscopic ureterotomy. Results All 8 patients successfully completed laparoscopic ureterotomy, without secondary injury or complication;and stone removal rate was 100%. Conclusion After ureteroscopic lithotripsy failure of ureteral calculi at the horizontal level of L5 to sacroiliac joint, the treatment of retroperitoneal laparoscopic ureterotomy is safe and effective, and is worthy to be popularized in qualified grass-roots units.
