院前急救对老年严重多发创伤患者预后的影响

摘　　要：目的探讨院前急救对老年严重多发创伤患者预后的影响。方法选取我院急诊科2016年2月至2017年2月收治的30例开展院前急救的老年严重多发伤患者作为研究组,选取我院同期收治的30例未开展院前急救的老年严重多发创伤患者作为对照组。对比分析两组患者的受伤至救治时间、入住ICU时间、住院时间、 6个月后的预后及并发症情况。结果研究组的受伤至救治时间、入住ICU时间及住院时间均明显短于对照组(P <0.05)。研究组的ARDS、 MODS、应激性溃疡、腹腔感染发生率均明显低于对照组(P <0.05)。研究组的预后良好+中残比例显著高于对照组(P <0.05)。结论老年人发生严重多发创伤后病情发展较为迅速,开展积极的院前急救可显著缩短患者的受伤至救治时间,减少严重并发症发生,提高患者预后水平。Objective To explore the influence of pre-hospital first aid on the prognosis of elderly patients with severe multiple trauma.Methods 30 cases of elderly patients with severe multiple trauma admitted to our emergency department from February 2016 to February 2017 were selected as the study group.Another 30 cases of elderly patients with severe multiple trauma without pre-hospital during the same period were selected as the control group.The time from trauma to treatment,ICU admission time,hospitalization time,prognosis 6months after trauma and complications were compared between the two groups.Results The time from injury to treatment,ICU admission time and hospitalization time of the study group were significantly shorter than those of the control group(P<0.05).The incidences of ARDS,MODS,stress ulcer and abdominal infection of the study group were significantly lower than those of the control group(P<0.05).The rate of good prognosis and moderate disability of the study group was significantly higher than that of the control group(P<0.05).Conclusions The condition of elderly patients with severe multiple trauma develops rapidly.Active pre-hospital first aid can significantly shorten the time from injury to treatment,reduce the severe complications,and improve the prognosis of patients.
