摘 要:目的调查肺炎克雷伯菌引起的下呼吸道感染的临床分布特点及耐药性,为肺炎克雷伯菌感染治疗提供抗生素选择理论依据。方法收集2016.08-2017.10年吉林大学第一医院下呼吸道感染患者送检的痰标本分离的3999株肺炎克雷伯菌,分析其临床分布及药敏结果。结果检出率高的科室前三位为神经外科、重症监护室和呼吸科,分别为506株(12.6%),488株(12.2%),462株(11.6%)。药敏试验结果表明肺炎克雷伯菌对氨苄西林高度耐药,耐药率达99%以上;对哌拉西林、头孢曲松、环丙沙星耐药率较低,分别为36.16&,30.63%,28.06%;对碳青霉烯类、阿米卡星等抗菌药物敏感,耐药率为0.86%,3.00%。结论肺炎克雷伯杆菌引起的下呼吸道感染见于临床各科室,掌握其临床科室分布差异性及耐药性,能够指导临床降低感染率及合理使用抗生素。Objective To investigate the clinical distribution and drug resistance of lower respiratory tract infection caused by klebsiella pneumoniae for better management of klebsiella pneumoniae infection.Methods 3999 strains of klebsiella pneumoniae were isolated form sputum sample collected from patients with lower respiratory tract infection from the first hospital of jilin university from August 2016 to October 2017,and analyzed their clinical distribute and sensitivity.Results Most of the strains were form neurosurgery(12.6%,506/3999),intensive care units(12.2%,488/3999),Department of Respiratory Medicine(11.6%,462/3999).The results of drug sensitivity test showed that Amost klebsiella pneumoniae was resistant to ampicillin(99%),Less than half of the klebsiella pneumoniae were resistant to piperacillin(36.16%),ceftriaxone(30.63%)and ciprofloxacin(28.06%).Only a few strain were resistant to carapnenems(0.86%),amikacin(3.00%)and other antimicrobial agents.Conclusion the infection of lower respiratory tract caused by klebsiella pneumoniae can be found in all clinical departments.
