摘　　要：目的分析中性粒细胞胞浆抗体(ANCA)检测应用于系统性红斑狼疮疾病(SLE)的诊断价值。方法选取医院2015年9月-2017年9月收治的SLE的患者62例,通过间接免疫荧光法(IIF)检测ANCA含量,对于阳性ANCA患者再以酶联免疫吸附法(ELISA)进行蛋白水解酶3(PR3)、抗髓过氧化物酶(MPO)含量的测定,另选取同期接受检查的51例健康群体经ELISA检测PR3、MPO含量的资料,比较SLE和健康群体ANCA阳性率差异,SLE活动性评估以SLE活动指数(SLEDAI)进行,探讨ANCA与SLEDAI的关系。结果SLE患者ANCA阳性检出率(37.10%)显著高于健康人群(1.96%)(P<0.05);ANCA阳性和ANCA阴性皮肤血管炎、肺间质变、补体C3或C4下降、肾功能受损、dsDNA抗体阳性有显著差异(P<0.05);ANCA阳性SLE患者SLEDAI≥10(73.91%)显著高于ANCA阴性(58.97%),SLEDAI<10(26.09%)显著低于ANCA阴性(41.03%)。结论对于SLE患者,可采取ANCA检测,其具有较为准确的阳性检出率,同时对SLE疾病活动性的判断有一定诊断意义。Objective To analyze the diagnostic value of ANCA for systemic lupus erythematosus(SLE).Methods From September 2015 to September 2017,62 patients with SLE were selected in our hospital.The content of ANCA was detected by indirect immunofluorescence assay(IIF),and the protein hydrolase 3(PR3),anti myeloperoxidase(MPO)was determined by Elisa for the patients with positive ANCA.In addition,51 healthy subjects who were examined in the same period were selected.PR3 and MPO content were examined by ELISA.The difference of ANCA positive rate between SLE and healthy subjects was evaluated by SLE activity index(SLEDAI).The relationship between ANCA and SLEDAI was discussed.Results The ANCA positive rate(37.10%)was significantly higher than that in healthy controls(1.96%)(P<0.05).There were significant differences in ANCA negative cutaneous vasculitis,pulmonary interstitial degeneration,decreased complement C 3or C 4,and impaired renal function and positive dsDNA(P<0.05).In SLE patients with positive SLEDAI≥10(73.91%)was significantly higher than that in patients with negative ANCA(58.97%)and SLEDAI<10(26.09%)was significantly lower than that in patients with negative ANCA(41.03%).Conclusion ANCA can be used in the detection of SLE,and it has a more accurate positive detection rate.At the same time,it has a value for the diagnosis of systemic lupus erythematosus.
