摘  要:目的研究微小RNA-499(miRNA-499)、心肌肌钙蛋白I(cTnI)、肌酸激酶同工酶(CK-MB)在急性心肌梗死(AMI)诊断中的价值。方法抽取本院2017年1月——2018年5月收治的53例AMI患者为观察组,同期体检健康人群45例为对照组。通过全自动实时分辨免疫荧光法测定血浆cTnI、CK-MB水平,实时荧光定量PCR(qRT-PCR)法测定血浆miRNA-499含量。比较二组平均血浆miRNA-499相对表达水平、cTnI、CK-MB水平及其阳性率;动态观察患者发病0~h、2~h、4~h、8~h、12~16 h血浆miRNA-499、cTnI、CK-MB水平变化;miRNA-499、cTnI、CK-MB之间相关性行Pearson分析;受试者工作特征曲线(ROC)分析miRNA-499诊断AMI曲线下面积(AUC)。结果观察组血浆miRNA-499相对表达水平、cTnI及CK-MB水平分别为(3.70±1.41)ng/ml、(24.02±6.38)ng/ml、(67.00±16.18)ng/ml,差异有统计学意义(P <0.05);观察组发病2~h血浆miRNA-499相对表达水平最高(4.30±1.21),血浆cTnI水平于发病4~h最高(24.86±4.10)ng/ml,CK-MB发病8~h最高,为(81.64±9.63)ng/ml;血浆miRNA-499相对表达水平与cTnI、CK-MB水平均呈正相关(均P<0.05);血浆cTnI、CK-MB阳性率观察组均显著高于对照组(均P<0.05);miRNA-499诊断AMI曲线下面积(AUC)为0.957;cTnI诊断灵敏度、特异度分别为83.02%、95.56%,CK-MB诊断灵敏度、特异度分别为75.47%、84.44%。结论 miRNA-499、cTnI、CK-MB诊断AMI各有优势,联合诊断价值更大。Objective To study the diagnostic value of microRNA-499(microRNA-499),cardiac troponin I(cTnI),creatine kinase isoenzyme(CK-MB)in acute myocardial infarction(AMI).Methods A total of fiftythree patients with AMI admitted to Dandong Central Hospital from January 2017 to May 2018 were included in the observation group.Forty-five healthy subjects at the same period were selected as the control group.Plasma cTnI and CK-MB levels were measured by automatictime-resolved fluoresecence immunoassay,the content of plasma miRNA-499 was determined by real-time quantitative PCR(qRT-PCR).The relative expression levels of average plasma miRNA-499,levels and positive rates of cTnI and CK-MB were compared between the two groups.Changes in levels of plasma miRNA-499,cTnI and CK-MB in the observation group in 0-2 hours,2-4 hours,4-8 hours,8-12 hours and 12-16 hours after onset were dynamically observed.Pearson analysis was performed to analyze the correlation among miRNA-499,cTnI and CK-MB.The receiver operating characteristic curve(ROC)was used to analyze area under the AMI curve(AUC)diagnosed with miRNA-499.Results The relative expression level of plasma microRNA-499,cTnI and CK-MB in the observation group were(3.70±1.41)ng/ml,(24.02±6.38)ng/ml,(67.00±16.18)ng/ml,respectively,with significant difference(P<0.05);the relative expression level of plasma microRNA-499 in the observation group was the highest(4.30±1.21),the cTnI level in the plasma was the highest((24.86±4.10)ng/ml)at the onset of 2-h,and the CK-MB level was the highest at the onset of 8-h.The positive rates of plasma cTnI and CKMB in the observation group were significantly higher than those in the control group(P<0.05);the area under the curve of MINA-499(AUC)was 0.957;the diagnostic sensitivity and specificity of cTnI were83.02%,95.56%,the diagnostic sensitivity and specificity of CK-MB were 83.02%,95.56%,respectively.75.47% and 84.44%respectively.Conclusion MiRNA-499,cTnI and CK-MB have their own advantages in the diagnosis of AMI,and the value of combined diag
