邓铁涛沐足方治疗难治性高血压的临床疗效

摘　　要：目的:评价邓铁涛沐足方治疗难治性高血压的疗效,明确其对平稳控制血压、改善血管硬化程度及对肾功能的保护作用。方法:纳入2014年12月—2016年12月本院住院部及心脏中心门诊的难治性高血压患者142例,随机分为治疗组及对照组。分别记录两组治疗前后的收缩压(SBP)、舒张压(DBP)、中医证候、AASI、生化指标进行分析。结果:血压控制方面,经治疗后,治疗组SBP、DBP在2周下降的幅度较大,后一直持续下降;而对照组在治疗4周后血压出现平稳状态。在治疗后第12周,治疗组与对照组SBP、DBP具有明显统计学差异,P<0.05;中医证候方面,治疗组71例中显效49例(69.0%),有效19例(26.8%),总有效率95.8%;对照组71例中显效38例(53.5%),有效22例(31.0%),总有效率84.5%,差异有显著性,P<0.05;生化指标方面,治疗组生化指标(Cr、CyS-C、UA)下降明显,差异具有统计学意义,P<0.05;经12周的治疗后,治疗组与对照组相比,治疗组AASI显著下降,差异具有统计学意义,P<0.05。结论:邓铁涛沐足方对于持续降低难治性高血压患者血压、改善患者临床症状、减少血液中尿毒素及降低AASI有显著作用,对改善血管硬化程度及保护靶器官具有一定的临床意义,值得推广应用。Objective:To evaluate the clinical efficacy of DENG Tietao′s foot bathing agent in the treatment of resistant hypertension,definite the protect effect of the blood pressure,improve the degree of vascular sclerosis and protect the renal function.Methods:From December 2014-December 2016 in our hospital,142 resistant hypertension patients were randomly divided into treatment group and control group.We recorded systolic pressure(SBP)and diastolic pressure(DBP),syndromes,ambulatory arterial stiffness index,biochemical indicators before and after treatment.Results:After treatment,the decrease of SBP and DBP in the treatment group was greater than that in the 2 weeks and then decreased continuously.In the control group,the blood pressure was stable after 4 weeks treatment.At 12 th week after treatment,the SBP and DBP of the treatment group and the control group were significantly different,P<0.05.The treatment group′s 71 cases had 49 cases markedly effective(69.0%),19 cases effective(26.8%)and the total effective rate was 95.8%.The control group′s 71 cases had 38 cases markedly effective(53.5%),22 cases effective(31.0%)and the total efficiency was 84.5%.The difference was significant,P<0.05.The biochemical indicators of the treatment group(Cr,CyS-C,UA)significantly decreased and the difference was statistically significant,P<0.05.After 12 weeks treatment,compared with the control group,the treatment group′s AASI decreased significantly and the difference was statistically significant,P<0.05.Conclusion:DENG Tietao′s foot bathing agent has significant effect on continuing to reduce blood pressure,improving clinical symptoms,reducing urinary toxins and AASI of resistant hypertension patients and has certain clinical significance on the improvement of arteriosclerosis and target organ protection.It is worthy of popularization and application.
