二甲双胍对1型糖尿病合并超重/肥胖患者胰岛素敏感性的分析

摘　　要：目的观察接受胰岛素治疗的超重/肥胖1型糖尿病(T1DM)患者联合使用二甲双胍治疗后胰岛素敏感性的变化。方法从超重/肥胖T1DM患者中,通过高胰岛素正葡萄糖钳夹试验筛选出5例胰岛素敏感性降低患者,比较患者在联用二甲双胍治疗前和治疗12周后体重指数(BMI)、糖化血红蛋白(HbA1c)、葡萄糖利用率(M值)的变化,评价葡萄糖敏感性有无改善。结果经过12周二甲双胍干预治疗,患者BMI、HbA1c水平均有不同程度下降,M值较前升高。结论使用胰岛素治疗的超重/肥胖、胰岛素敏感性降低的T1DM患者联合使用二甲双胍12周后,其胰岛素敏感性可获得不同程度的改善。Objective To observe the changes of insulin sensitivity in overweight/obese type 1 diabetes patients with the treatment of insulin combined metformin. Methods From overweight/obese patients with type 1 diabetes,5 patients with impaired insulin sensitivity were screened by high insulin positive glucose clamp test.Body mass index(BMI),glycosylated hemoglobin(HbA1c)and the glucose utilization rate(M value)were compared before and 12 weeks after the metformin treatment.Results After 12 weeks treatment of metformin,the BMI and HbA1c were reduced,mean M value was increased and the glucose utilization rate was improved. Conclution In overweight/obese type 1 diabetes patients with insulin treatments,the insulin sensitivity was improved after 12 weeks of combined use of metformin.
