摘　　要：双向转诊制度是促进医疗资源合理分配、促进患者健康的重要举措,但如今却出现了"上转容易下转难"的典型特征,而向下转诊患者延续性护理是促进双向转诊中患者下转实施的重要推动力之一。本文对我国双向转诊模式中向下转诊患者延续性护理实施的必要性、内涵、主要服务形式、应用效果、问题分析等方面进行综述,以期为建立适合患者双向转诊的护理保障体系提供依据。Two-way referral system is an important measure to promote the rational distribution of medical resources and promote the health of patients, but now there is a typical characteristic of "referrals up easy but downward referral difficult". while the transitional care of downward referral patients is one of the important impetus to promote the implementation of downward referral, this paper summarizes the necessity, connotation, main service forms, application effects and problem analysis status of the transitional referral of patients in the two-way referral mode in our country, in order to provide the basis for the establishment of a nursing security system suitable for two-way referral of patients.
