维持性血透患者透析中收缩压变异性及其影响因素的探讨

摘　　要：目的初步探讨维持性血液透析(MHD)患者透析过程中收缩压变异性(SBPV)及相关影响因素。方法选择MHD患者202例,利用臂式电子血压仪监测和记录连续30d内所有血透过程中的血压值,利用变异系数(CV)表示患者血透过程中的收缩压变异性(SBPV),同时收集患者一般临床资料和实验室检查数据。根据SBPV均值将患者分为高SBPV组和低SBPV组,分别对两组患者的临床资料与实验室数据进行分析。结果高SBPV组与低SBPV组比较,患者年龄更大(P=0.001)、平均舒张压更低(P=0.001)、低血压发生次数更多(P=0.001)、透析前血清钠更高(P=0.043)、白蛋白更低(P=0.012)、C-反应蛋白更高(P=0.015)。多因素线性回归分析显示患者年龄、血清钠、钙磷乘积和低血压发生次数与SBPV呈正相关。结论MHD患者透析中SBPV与年龄、血清钠、钙磷乘积和低血压发生次数呈正相关。Objective To explore the characteristics and related factors of systolic blood pressure variability(SBPV) during dialysis in maintenance hemodialysis (MHD) patients. Methods We enrolled 202 MHD patients, using the electronic sphygmomanometer arm to monitor and record the blood pressure during dialysis in 30 consecutive days. The coefficient of variation (CV) was used to calculate systolic blood pressure variability (SBPV), and the clinical data and laboratory data of the patients were collected at the same time. The related factors of SBPV and the characteristics were analyzed. Results High SBPV group compared with the low SBPV group , older age (P=0.001), lower average DBP (P=0.001), more frequent hypotension (P=0.001) , higher serum sodium (P=0.043), lower serum albumin (P=0.012), higher C-reactive protein (P=0.015), the differences were statistically significant. Multivariate analysis demonstrated that age, serum sodium, calcium-phosphorus products and the frequency of hypotension were positively correlates with SBPV. Conclusions Age, serum sodium, calcium-phosphorus products and the frequency of hypotension are positively correlates with intradialytic SBPV in the MHD patients.
