自拟加味瓜蒌薤白半夏汤对痰热蕴肺型慢性阻塞性肺疾病急性加重期患者炎症因子及凝血功能的影响

摘　　要：【目的】观察自拟加味瓜蒌薤白半夏汤对痰热蕴肺型慢性阻塞性肺疾病(简称慢阻肺)急性加重期患者炎症因子及凝血功能的影响。【方法】将132例痰热蕴肺型慢阻肺急性加重期患者随机分为试验组和对照组,每组各66例。对照组给予常规西药治疗,试验组在对照组基础上加用加味瓜蒌薤白半夏汤治疗,疗程均为2周。观察2组患者治疗前后肺功能指标、炎症因子、凝血功能指标的变化情况,并评价2组临床疗效。【结果】(1)治疗2周后,试验组总有效率为74.24%,对照组为45.45%,试验组疗效明显优于对照组(P<0.01)。(2)治疗后,2组患者的肺功能指标、炎症因子、凝血功能指标均较治疗前明显改善(P<0.01),且除凝血酶时间(TT)外,试验组对各项肺功能指标、炎症因子、凝血功能指标的改善作用均优于对照组,差异均有统计学意义(P<0.01)。【结论】自拟加味瓜蒌薤白半夏汤能有效提高痰热蕴肺型慢阻肺急性加重期患者临床疗效,降低患者炎症因子水平,改善患者肺功能和凝血功能。Objective To explore the effects of self-prescribed modified Gualou Xiebai Banxia Decoction(GXBD)on inflammatory factors and blood coagulation function of patients at acute exacerbation period of chronic obstructive pulmonary disease(COPD)with the syndrome of phlegm-heat accumulating lung.Methods A total of132 qualified patients were randomized into trial group and control group,66 cases in each group.Both groups were given conventional western medicine treatment,and additionally the trial group was given modified GXBD orally,the treatment lasting 2 weeks.Before and after treatment,lung function indexes,inflammation factors,and blood coagulation function indexes in the two groups were observed,and the curative effect of the two groups was compared after treatment.Results(1)After treatment for 2 weeks,the total effective rate in the trial group was74.24%and that in the control group was 45.45%,the effect of the trial group being superior to that of the control group(P<0.01).(2)After treatment,lung function indexes,inflammation factors,and blood coagulation function indexes in the two groups were much improved(P<0.01 compared with those before treatment);except for the thrombin time(TT),the improvement of the above indexes in the trial group was superior to that of the control group(P<0.01).Conclusion Modified GXBD is effective on enhancing the clinical efficacy of patients at acute exacerbation of COPD with the syndrome of phlegm-heat accumulating lung by decreasing the levels of inflammatory factors and improving the lung function and coagulation function of the patients.
