高分化肿块型肝内胆管细胞癌的MRI特征

摘　　要：目的:分析高分化肿块型肝内胆管细胞癌的MRI表现,以提高影像认识。方法:回顾性收集33例经手术病理证实的肝内胆管细胞癌,根据其病理分级不同将其分为高分化组(n=11)及非高分化组(n=22),对比分析2组MRI表现差异。结果:高分化肿块型肝内胆管细胞癌多为类圆形肿块(10/11,90.9%),边界清晰(11/11,100.0%),非高分化组多为边缘不清晰(14/22,63.6%)、不规则软组织肿块(16/22,72.7%),两者比较有统计学意义(P<0.05);增强扫描,高分化组以Ⅰ型及Ⅱ型强化为主,非高分化组以Ⅱ型及Ⅲ型为主,与非高分化组相比有统计学意义(P<0.05);高分化组DWI靶征6例(6/11,54.5%),伴随征象3例(3/11,27.3%),与非高分化组相比有统计学意义(P<0.05)。结论:高分化肿块型肝内胆管细胞癌多表现为类圆形、边界清晰的软组织肿块,强化模式多为Ⅰ型及Ⅱ型,伴随征象少见,DWI可见靶征。Objective: To discuss the MRI signs of high differentiation intrahepatic cholangiocarcinoma in order to improve the understanding of this disease. Methods: The MRI signs and pathological data of 33 intrahepatic cholangiocarcinoma were analyzed retrospectively. According to different pathological grading, all patients were divided in two groups: high differentiation group(n =11) and non-high differentiation group(n =22). The MRI appearances were compared between two groups. Results: High differentiation intrahepatic cholangiocarcinoma showed circular mass(10/11, 90.9%), clear border(11/11, 100.0%), compared to 63.6% and 72.7% in non-high differentiation group, in which has statistically significance( P <0. 05). In high differentiation group, the tumor often showedⅠand Ⅱ strengthening type, compared to Ⅱ and Ⅲ enhancement mode in non-high differentiation group, there were statistically significance( P <0. 05). There were 6 cases(6/11,54.5%) showed DWI target appearance and 3(3/11,27.3%) cases accompanying signs in high differentiation group. There were statistically significance( P <0. 05). Conclusion: High differentiation intrahepatic cholangiocarcinoma often shows circular, clear border mass, Ⅱ and Ⅲ enhancement mode and DWI target sign;accompanying signs is likely rare.
