经皮微创椎板间入路椎间孔镜下椎管减压术对老年腰椎侧隐窝狭窄症的效果分析

摘　　要：目的研究经皮微创椎板间入路椎间孔镜下椎管减压术对老年腰椎侧隐窝狭窄症的临床疗效。方法纳入2014-06-2016-06,治疗的58例老年腰椎侧隐窝狭窄症患者,单节段狭窄44例,双节段14例,均采用经皮微创椎板间入路椎间孔镜下椎管减压术治疗。术后随访24～40个月,平均(30.45±3.94)月,观察其手术疗效和术后并发症情况。结果 58例患者均成功完成手术,手术平均时间(65.22±4.98)min;末次随访时,手术优良率93.10%;术后1个月、6个月和末次随访的VAS腰痛和下肢痛评分以及ODI评分均显著低于治疗前(P<0.05);末次随访骨性与软性侧隐窝角均显著高于术前(P<0.05)。结论经皮微创椎板间入路椎间孔镜下椎管减压术治疗老年腰椎侧隐窝狭窄症,能显著改善患者腰腿疼痛及活动功能,手术安全有效。Objective To study the clinical effect of percutaneous minimally invasive interlaminar approach for decompression of spinal canal under intervertebral foramen for lumbar lateral recess stenosis in the elderly.Methods Fifty-eight elderly patients with lumbar lateral recess stenosis treated in our hospital from June 2014 to June 2016 were included in this study.There were 44 patients with single-segment stenosis and 14 patients with double segment.All patients underwent percutaneous minimally invasive surgery. Inter-plate intervertebral foramen underwent decompression of the spinal canal.The patients were followed up for 24 to 40 months with an average of (30.45±3.94)months to observe the curative effect and postoperative complications.Results All the 58 patients successfully completed the operation,the average operation time was (65.22±4.98)min.The excellent and good rate was 93.10% at the last follow-up.At 1 month,6 months after operation and the last follow-up,the VAS scores of low back pain,lower limb pain and ODI scores were significantly lower than before treatment (P<0.05).The angle of soft and osseous lateral recess at the last follow-up was significantly higher than before operation (P<0.05).Conclusion Percutaneous minimally invasive interlaminar decompression for the treatment of lumbar lateral recess stenosis in the elderly can significantly improve the pain and motor function of patients with lumbar and leg,the operation is safe and effective.
