摘　　要：目的:构建静脉用药调配中心质量指标体系,为提高静脉用药调配中心管理质量和管理效能提供评估依据。方法:统计2012-2014年度静脉用药调配中心交班记录中的所有错误,运用柏拉图确定5项核心质量指标,分别为审方、贴签、排药、调配和复核包装。基于历史数据确定5项核心指标的基准数值,每月进行数值比对,对低于基准值的项目进行专项整改活动。结果:建立了静脉用药调配中心数字化的质量管理体系,强化了运行管理,使审方、贴签、排药、调配和复核包装错误发生率从2015年起呈显著下降。结论:静脉用药调配中心基于历史数据将5项核心质量指标数字化,并依据该指标对科室的日常工作进行质量监管工作并持续改进,减少了错误发生率,使静脉用药调配中心的管理更加科学高效。OBJECTIVE To establish the quality management system and provide an assessment basis for improving management quality and management effectiveness of PIVAS. METHODS All the errors in the record of PIVAS between 2012 and 2014 were counted, and the core quality indicators were determined by Pareto Chart, including prescription checking, labeling, drug dispensing, drug deploying, and rechecking packaging. The benchmark values of the five core indicators were determined based on historical data, and numerical comparisons were performed monthly to carry out special rectification activities for projects below the benchmark value. RESULTS The digital quality management system of PIVAS was established, and the operation management was strengthened, which caused the error rate of prescription checking, labeling, drug dispensing, drug deploying, and rechecking packaging to decline significantly from 2015 to 2017.CONCLUSION Based on historical data, PIVAS digitizes five core quality indicators, and conducts quality control for the daily work of the department based on this indicator, and continuously improves, reducing the incidence of errors and making the management of PIVAS more scientific and efficient.
