血清TPSA检测在慢性前列腺炎临床分型诊断中的价值

摘　　要：目的 探讨血清总前列腺特异抗原水平对慢性前列腺炎临床分型的诊断价值.方法 将148例慢性前列腺炎患者按病理分类分为Ⅱ型组(48例) 、Ⅲ A 型组(56例) 、ⅢB型组(44例) ,选取本院同期健康体检者50名设为对照组.于治疗前及治疗4周末比较各组血清总前列腺特异抗原水平及阳性率.结果 以时间因素以及时间因素和分组的交互作用均有统计学意义(P<0 .01) .治疗前Ⅱ型组、ⅢA型组、ⅢB型组血清总前列腺特异抗原水平均显著高于对照组(P<0 .05) ,且Ⅱ型组显著高于ⅢA型组、ⅢB型组(P<0 .05) ,ⅢA型组与ⅢB型组比较差异无统计学意义(P> 0 .05).治疗后 Ⅱ型组血清总前列腺特异抗原水平显著低于治疗前(P<0 .05) ,ⅢA型组、ⅢB型组变化无统计学意义(P>0 .05) ,Ⅱ型组显著低于Ⅲ A型组、ⅢB型组(P<0 .05).治疗前Ⅱ型组、ⅢA型组、ⅢB型组阳性率显著高于对照组(P<0 .01) ,且Ⅱ型组高于ⅢA型组、ⅢB型组(P<0 .05) .治疗后Ⅱ型组、ⅢA型组阳性率显著下降(P<0 .05) ,各组间阳性率比较差异无统计学意义(P>0 .05).结论 血清总前列腺特异抗原水平检测对慢性前列腺炎患者的临床分型诊断具有一定指导价值.Objective To investigate the diagnostic value of serum total prostate specific antigen level for clinical classification of chronic prostatitis.Methods According to the pathological classification,148 patients with chronic prostatitis were divided into typeⅡgroup(48 cases),typeⅢA group(56 cases)and typeⅢB group(44 cases),50 healthy people in the same period were selected as the control group.Serum total prostate specific antigen levels and positive rates were compared before treatment and at 4 weeks after treatment.Results There was significant difference in time factor and the interaction of time factor and grouping(P<0.01).The serum total prostate specific antigen levels in typeⅡgroup,typeⅢA group and typeⅢB group before treatment were significantly higher than those in the control group(P<0.05),typeⅡgroup was significantly higher than typeⅢA group and typeⅢB group(P<0.05),there was no significant difference between typeⅢA group and typeⅢB group(P>0.05).The serum total prostate specific antigen level of typeⅡgroup was significantly lower than that before treatment(P<0.05),there was no significant difference between typeⅢA group and typeⅢB group(P>0.05),typeⅡgroup was significantly lower than typeⅢA group and typeⅢB group(P<0.05).The positive rates of typeⅡgroup,typeⅢA group and typeⅢB group before treatment were significantly higher than that of the control group(P<0.01),and typeⅡgroup was significantly higher than typeⅢA group and typeⅢB group(P<0.05).The positive rate of typeⅡgroup and typeⅢA group decreased significantly after treatment(P<0.05),there was no significant difference in the positive rate among the groups(P>0.05).Conclusion The detection of serum total prostate specific antigen level has certain guiding value for the clinical classification of patients with chronic prostatitis.
