脊柱外固定器联合经皮椎体内植骨术治疗胸腰椎骨折疗效观察

摘　　要：目的 探讨脊柱外固定器联合经皮椎体内植骨术治疗胸腰椎骨折临床疗效.方法 对120例胸腰椎骨折患者给予脊柱外固定器联合经皮椎体内植骨术治疗,比较治疗前及治疗3个月、6个月患者脊柱后突Cobb's角及椎体前后缘高度的压缩率,并测量骨折椎体前后缘高度及椎管狭窄率.根据Frankel脊髓损伤分级对患者的神经功能损伤情况进行判定.结果 治疗后3个月及6个月本组患者脊柱后突Cobb's角,椎体前缘及后缘高度的压缩率及椎管狭窄率均较治疗前显著改善(P<0 .01) .治疗后3个月及6个月患者的神经功能均达到B级以上,治疗后3个月E级患者80 .0%,治疗后6个月E级患者72 .5%,均较治疗前显著改善( P<0 .01) .结论 脊柱外固定器联合经皮椎体内植骨术治疗胸腰椎骨折疗效显著,能够有效维持损伤脊柱的稳定性,保证其神经功能的完整,值得推广使用.Objective To investigate the clinical effect of external spinal fixation combined with percutaneous vertebral body bone graft for thoracolumbar fractures.Methods 120 cases of patients with thoracolumbar fractures were given external spinal fixation combined with percutaneous vertebral body bone graft treatment.The ithyokyphosis Cobb's angle,compression rate of anterior and posterior edge height of the two groups before treatment and 3,6 months after treatment were compared,and measure the anterior and posterior edge height and spinal stenosis rate of fractured vertebral body.The neurological impairment of the patient was determined according to the Frankel spinal cord injury grade.Results 3,months and 6 months after treatment,the ithyokyphosis Cobb's angle,compression rate of anterior and posterior edge height,and spinal stenosis rate of the group were significantly improved compared with those before treatment (P<0.05).The neurological function of patients at 3 months and 6 months after treatment reached grade B or higher.80.0% of patients with E grade at 3 months after treatment,72.5% of patients with grade E at 6 months after treatment,all significantly improved compared with those before treatment (P<0.01).Conclusion External spinal fixation combined with percutaneous vertebral body bone graft in the treatment of thoracolumbar fractures has significant effect,it can effectively maintain the stability of the injured spine and ensure the integrity of its nerve function,which is worthy of popularization.
