三枚空心钉治疗股骨颈骨折研究进展

摘　　要：股骨颈骨折是指股骨颈骨质的连续性遭到破坏,由于其特殊的解剖结构,骨折时常常因损伤其供应血管,从而导致骨折不愈合、股骨头坏死等情况的发生。在其治疗方法的选择上如何有效避免骨折不愈合、股骨头坏死一直是研究的重点和难点。股骨颈骨折的治疗包括保守治疗、内固定手术治疗、人工髋关节置换等方法。随着内固定物不断更新及人工髋关节置换的应用,股骨颈骨折的手术治疗已取代了保守治疗,而通过内固定修复股骨颈骨折并尽可能保留自身股骨头是当前手术治疗的主流,三枚空心钉内固定治疗股骨颈骨折是目前比较推崇的一种内固定方式。近几年来,在三枚空心钉治疗股骨颈骨折的基础上,又有了新的进展,尤其是空心钉F技术(The biplane double-supported screw fixation method)的应用,更加突出了三枚空心钉治疗股骨颈骨折的优点,有望在今后治疗移位并骨质疏松型股骨颈骨折常规应用。本文旨在对三枚空心钉内固定治疗股骨颈骨折的基础上新的应用及相关进展进行综述。Femoral neck fracture refers to the destruction of the continuity of the femoral neck bone.Due to the special anatomical structure of femoral neck,the supply of blood vessels is often destructed after the fracture,which may result in fracture nonunion,femoral head necrosis,etc.How to choose a therapeutic method which can effectively avoid these consequences has been the focus of study in the treatment of femoral neck fracture.The therapeutic methods of femoral neck fracture include conservative treatment, internal fixation surgery,artificial hip replacement,etc.With the constant updating of internal fixtures and the application of artificial hip replacement,surgical treatment of femoral neck fractures has gradually replaced conservative treatment.To repair femoral neck fractures by internal fixation and try every means to keep the femoral heads is the mainstream of current surgical treatments.Hollow compression nail fixation is currently a well-recognized method of internal fixation.In recent years,further achievement has been scored on the basis of three hollow compression nails for the treatment of femoral neck fractures.In particular,the application of cannulated screw F technology has highlighted its advantages.It is expected to be applied in the future to treat displaced and osteoporotic femoral neck fractures.This article aims to review the new applications and related advances in the treatment of femoral neck fractures with three hollow compression nails.
