丙种球蛋白辅助盐酸氨溴索静注对新生儿肺炎血气指标和免疫功能的影响

摘　　要：目的探讨丙种球蛋白(IG)辅助盐酸氨溴索静脉滴注(静注)对新生儿肺炎血气指标和免疫功能的影响。方法选取2014年7月‐2017年5月该院收治的92例新生儿肺炎患儿,随机分为盐酸氨溴索组(n=46)和联合组(n=46)。盐酸氨溴索组患儿仅单独使用盐酸氨溴索静注治疗,联合组患儿在盐酸氨溴索组基础上给予IG静注治疗。分析比较两组患儿治疗前后的血气指标和免疫功能指标的变化。结果治疗后,联合组患儿动脉血氧分压(PaO_2)、氧合指数大于盐酸氨溴索组,二氧化碳分压(PaCO_2)小于盐酸氨溴索组(P <0.05);联合组患儿免疫球蛋白A(IgA)、免疫球蛋白G(IgG)及免疫球蛋白M(IgM)水平高于盐酸氨溴索组(P <0.05);联合组患儿治疗有效率高于盐酸氨溴索组(100.00%vs. 89.13%, P <0.05)。结论 IG辅助盐酸氨溴索静注治疗能显著改善新生儿肺炎患儿的血气指标和免疫功能,提高患儿临床疗效。【Objective】To investigate the effect of gamma globulin-assisted ambroxol hydrochloride injection on neonatal pneumonia blood gas index and immune function.【Methods】Ninety-two neonatal pneumonia patients who were admitted to our hospital from July 2014 to May 2017 were randomly divided into ambroxol hydrochloride group(n =46) and combination group (n =46). Patients in the ambroxol hydrochloride group were treated with ambroxol hydrochloride intravenously alone. Patients in the combination group were given gamma globulin intravenous injection on the basis of the ambroxol hydrochloride group. Changes in blood gas parameters and immune function before and after treatment were analyzed and compared in both groups【.Results】After treatment, the arterial partial pressure of O2(PaO2), oxygenation index were greater, and the arterial partial pressure of CO2(PaCO2) was smaller in the combination group than those in the ambroxol hydrochloride group(P <0.05);the immunoglobulin-A(IgA), immunoglobulin-G(IgG), and immunoglobulin-M(IgM) levels of the combination group were higher than those of the ambroxol hydrochloride group(P <0.05). The effective rate of the combination group was higher than that of the ambroxol hydrochloride group(100.00% vs. 89.13%, P <0.05).【Conclusion】Gamma globulin-assisted intravenous ambroxol hydrochloride injection can significantly improve the blood gas index and immune function of neonates with pneumonia and improve the clinical efficacy.
