小金片联合过氧化苯甲酰凝胶治疗痰瘀互结型聚合型痤疮的临床效果及安全性

摘　　要：目的比较小金片联合过氧化苯甲酰凝胶与丹参酮胶囊联合过氧化苯甲酰凝胶治疗聚合性痤疮(痰瘀互结型)的临床效果及安全性。方法选择首都医科大学附属北京安贞医院2015年6月至2017年6月收治的聚合型痤疮患者92例,采用随机数字表法分为观察组和对照组,各46例。观察组给予小金片口服联合过氧化苯甲酰凝胶外涂,对照组给予丹参酮胶囊口服联合过氧化苯甲酰凝胶外涂,疗程均为4周。比较2组患者的临床疗效、治疗前后皮损数量和生活质量量表评分以及复发情况。结果观察组失访1例、退出治疗2例,对照组失访2例,退出治疗1例,最终纳入研究各43例。观察组治疗4周总有效率高于对照组[90. 7%(39/43)比69. 8%(30/43)],差异有统计学意义(χ~2=5. 71,P <0. 05)。治疗后,对照组与观察组患者皮损数量均少于治疗前,且观察组皮损数量少于对照组[(14±5)个比(20±6)个],差异均有统计学意义(均P <0. 05)。治疗后,2组患者的自我感知、情感功能和社会功能评分均较治疗前升高,且观察组高于对照组(均P <0. 05)。停药后1个月随访,观察组痊愈者的复发率低于对照组[5. 0%(1/20)比30. 8%(4/13)],差异有统计学意义(χ~2=9. 76,P <0. 05)。结论小金片联合过氧化苯甲酰凝胶治疗聚合型痤疮(痰瘀互结型)临床疗效显著、安全性好且痊愈者停药后复发率低。Objective To observe the efficacy and safety of Xiaojin tablets or tanshinone capsules combined with benzoyl peroxide gel in treatment of acne conglobata(phlegm and blood stasis type). MethodA total of 92 cases of acne conglobata admitted to Beijing Anzhen Hospital, Capital Medical University from June 2015 to June 2017 were randomly divided into observation group and control group, with 46 cases in each group. The observation group took Xiaojin tablets and the control group took tanshinone capsules, and both groups were given external use of benzoyl peroxide gel. Clinical efficacy, number of skin lesions, quality of life and relapse rate were analyzed after 4 weeks of treatment. Results There was 1 lost case and 2 cases of withdrawal from treatment in observation group and 2 lost cases and 1 withdrawal in control group;43 cases were included in each group. Total effective rate in observation group was higher than that in control group[90.7%(39/43) vs 69.8%(30/43)](χ^2=5.71, P<0.05). Number of skin lesions lessened after treatment and the number in observation groups was less than that in control group[(14±5) vs (20±6)](P<0.05). After treatment, scores of self-perception, emotional function and social function were improved compared to those before treatment;the scores in observation group were higher than those in control group(all P<0.05). During follow-up in 1 month after cessation, the recurrence rate of cured patients in observation group was lower than that in control group[5.0%(1/20) vs 33.3%(4/12)](χ^2=9.76, P<0.05). Conclusion Xiaojin tablets combined with benzoyl peroxide gel treating acne conglobata shows good clinical efficacy, safety and low recurrence rate.
