摘　　要：目的:为了解桐乡市60岁以上老年人的认知功能障碍状况及影响因素,为预防老年人认知功能障碍提供科学依据。方法:采用整群抽样方法在桐乡市石门镇选择符合条件的老年人进行问卷调查和体格检查。结果:1 566名社区老年人中MMSE平均得分(24. 55±5. 15)分,有认知功能障碍者198名,占11. 96%。多因素Logistic回归分析显示高龄(OR=1. 350,95%CI:1. 166～1. 738,P=0. 006)、无配偶(OR=1. 477,95%CI:1. 026～2. 126,P=0. 036)等是60岁以上老年人认知功能障碍独立危险因素,富裕(OR=0. 252,95%CI:0. 113～0. 561,P=0. 001)、无伤害(OR=0. 285,95%CI:0. 164～0. 496,P=0. 000)、无糖尿病(OR=0. 498,95%CI:0. 280～0. 886,P=0. 018)是60岁以上老年人认知功能障碍的保护因素。结论:建议政府对老年人的认知功能状况进行早期筛查和重点干预。Objective To explore the cognitive dysfunction and influencing factors of the aged over 60 in Tongxiang city,and to provide scientific basis for preventing cognitive dysfunction of the elderly. Methods Cluster sampling method was used to carry out a questionnaire survey and physical examination among the eligible aged in Shimen town of Tongxiang city. Results Among the 1566 community elderly,mini mental state examination( MMSE) averaged score was( 24. 55 ± 5. 15) and 198 of them had cognitive impairment( 11. 96%). Multivariable Logistic regression analysis revealed that advanced age( OR =1. 350,95% CI: 1. 166 ~ 1. 738,P = 0. 006),without spouse( OR = 1. 477,95% CI: 1. 026 ~ 2. 126,P = 0. 036) were independent risk factors for cognitive dysfunction in the elderly over 60. Wealth( OR = 0. 252,95% CI: 0. 113 ~ 0. 561,P =0. 001),without harm( OR = 0. 285,95% CI: 0. 164 ~ 0. 496,P = 0. 000),without diabetes( OR = 0. 498,95% CI: 0. 280~ 0. 886,P = 0. 018) were the protective factors for cognitive dysfunction of the aged over 60. Conclusions It is suggested that the government should carry out early screening and key intervention in the cognitive function of the elderly.
