基于综合评估模式的老年冠心病患者干预效果评价

摘　　要：目的:探讨老年冠心病患者实施老年综合评估(CGA)模式干预的效果。方法:选取90例老年冠心病(CHD)患者,依据随机数字表法随机分为对照组和干预组,各45例;对照组给予常规心内科护理,而干预组则在此基础上给予CGA模式干预;于出院前对2组患者的治疗依从性、心脏不良事件、护理不良事件及生活质量进行对比分析。结果:干预后,干预组的按时吃药、配合治疗、遵从医师建议等依从性好于对照组(χ21=15. 315、χ22=17. 764、χ23=19. 994,P <0. 01);干预组的心脏不良事件、护理不良事件发生率分别为8. 9%、6. 7%,分别低于对照组的33. 3%、28. 9%(χ21=8. 073,χ22=7. 601,P <0. 01);干预组的生理功能、精神健康、精力状况及社会功能等评分均高于对照组(t1=10. 201、t2=10. 516、t3=10. 234、t4=7. 861,P <0. 01)。结论:老年冠心病患者实施CGA模式干预后,可提高治疗依从性,降低心脏和护理不良事件发生率,改善患者的生活质量。Objective To explore the effect of comprehensive geriatric assessment( CGA) model on elderly patients with coronary heart disease( CHD). Methods Ninety cases of CHD were randomly assigned into control group and intervention group according to random number table,with 45 cases in each group. The control group was given conventional cardiologic nursing,while the intervention group received an additional CGA model intervention. The indexes of treatment compliance,adverse cardiac events,adverse nursing events and life quality were compared between the two groups before hospital discharge. Results After intervention,the treatment compliance including taking medicine on time,coordination during treatment and following the doctor’s advice in the observation group were all better than the control group( χ^21= 15. 315,χ^22=17. 764,χ^23= 19. 994,P < 0. 01). The incidence rates of adverse cardiac events and adverse nursing events in the intervention group were 8. 9% and 6. 7%,both lower than the control group( 33. 3% and 28. 9%,χ^21= 8. 073,χ^22= 7. 601,P <0. 01). The scores of physiologic function,emotional health,vigour status and social function were all higher than the control group( t1= 10. 201,t1= 10. 516,t3= 10. 234,t4= 7. 861,P < 0. 01). Conclusions After the intervention of CGA model,the elderly CHD patients could raise treatment compliance,cut down adverse cardiac events and adverse nursing events,and improve life quality.
