两种腕部小切口手术治疗腕管综合征的疗效比较

摘　　要：目的对比两种腕部小切口手术治疗腕管综合征的临床疗效以及并发症。方法选择100例腕管综合征住院病例,随机分成两组,每组各50例,分别行腕掌侧小横切口和小纵切口手术,采用Kelly等评定分级法评价两点辨别觉的改善、手术时间及并发症的发生情况,对两组术式的疗效进行分析比较。结果两组患者术后1,3个月疗效比较,差异无统计学意义(P>0.05)。3个月后比较两组间两点辨别觉,差异无统计学意义(P>0.05);两组患者均无神经损伤的发生,瘢痕痛、手术及住院时间差异也无统计学意义(P>0.05)。结论腕掌侧小切口减压治疗腕管综合征,手术操作简单,疗效确切,并发症少,患者满意度高,适合各级医院广泛开展。横、纵小切口手术疗效无明显差异,具体术式的选择可根据术者个人熟练程度及设备条件。Objective To compare the clinical curative effect and complications of 2 kinds of small incisions in wrist to treat carpal tunnel syndrome.Methods One hundred cases of carpal tunnel syndrome in the hospital were chosen and randomly divided into 2 groups,50 cases in each group,were treated by small transverse incision and small longitudinal incision operation.Postoperative results were assessed using Kelly's standerd,two-point discrimination,operative time and complications.Results In the 2 groups,there was no significant difference between 1 and 3 months after operation(P>0.05),three months later,the differences were not statistically significant(P>0.05).There was no neurological damage in the two groups,and there was no significant difference in scar pain,operative time and hospital stay(P>0.05).Conclusion The operation is simple with good curative effect,few complication,high satisfaction of patients.It is suitable for all levels of hospital to carry out widely.There is no significant difference in the operative result of transverse and longitudinal incisions,and the choice can be based on individual preference and equipment conditions.
