摘　　要：目的探讨甲襞下切开结合甲半月钻孔引流治疗手外伤术后健指甲根炎的临床疗效。方法 2013年9月-2017年12月,对24例34指手外伤术后健指发生甲根炎的病例,采用甲襞下切开结合甲半月钻孔引流的方法进行治疗,根据观察指标判断术后疗效。结果所有病例经甲襞下切开结合甲半月钻孔引流治疗后全部治愈。新生指甲完全覆盖甲床,外观生长良好,无畸形。术后随访4～15个月,平均5.3个月,甲根无炎症复发,指甲外观平滑匀称,质地坚韧,甲体与甲床紧密结合,指端无疼痛、感觉异常等症状。根据吕桂欣等方法进行疗效评定:优32指,良2指,优良率为100%。通过Michigan手概况调查表评定患者对术后手指外观的满意度,结果显示:非常满意22例,满意2例。结论应用甲襞下切开结合甲半月钻孔引流的方法治疗手外伤术后健指甲根炎,方法简单,易于推广,临床效果良好。Objective To explore the clinical effect of nailfold incision combined with lunula trephination and drainage on the healthy nail root inflammation after hand trauma.Methods From September 2013 to December 2017,our department treated 24 cases of 34 fingers with nail root inflammation after hand trauma with nailfold incision combined with lunula trephination and drainage,according to the observation index,the curative effect of 24 patients was judged.Results All the patients in this group were cured by the nailfold incision combined with lunula trephination and drainage.The new nails completely covered the nail beds,with good appearance and no deformity.Postoperative follow-up of 4~15 months,with an average of 5.3 months,all the nail roots had no recurrence of inflammation,and nails were smooth,symmetrical,and tough,and the nail badies had been closely integrated with their nail beds,and all the fingers had no symptoms such as pain,paresthesia and so on.According to Lv guixin and others'methods to evaluate the efficacy:the result was excellent in 32 fingers,good in 2 fingers,and excellent and good rate was 100%.Using the Michigan hand survey to assess the patients'satisfaction with the appearance of postoperative fingers.The results showed that 22 patients were very satisfactory and 2 patients were satisfactory.Conclusion This method is simple,easy to be popularized and effective to treat health nail root inflammation after hand trauma by nailfold incision combined with lunula burring hole and drainage.
