摘  要:目的探讨动静脉畸形(AVM)与静脉畸形(VM)的超声声像图特征。方法回顾性分析84例AVM、85例VM患者超声资料。结果两者均为囊实相间混合回声、回声不均、内见数个管腔样无回声区,呈网格状或蜂窝状。两者深度、无回声区内径、静脉峰值具有统计学意义(均P<0.001),且AVM组均显著高于VM组。两者形状、边界具有统计学意义(均P<0.001)。AVM无钙化,部分VM见钙化(16%)。AVM彩色多普勒(CDFI)示无回声区内充满血流信号,呈五彩镶嵌状。频谱多普勒(PW)示所有病例无回声区内均測及动脉频谱,多呈高速均为低阻型,其中大部分仅測及动脉频谱,少部分除了測及动脉频谱外,还可測及静脉频谱,且以动脉频谱为主;VM CDFI示无回声区内见血流信号,挤压后血流信号增多,出现闪动现象,挤压试验阳性。PW示无回声区内测及静脉频谱,呈低速。结论 AVM与VM在超声图像上有一定相似性,但在CDFI及PW上有显著差异,对两种疾病的鉴别诊断具有重要的临床价值。Objective To investigate the ultrasonographic features of arteriovenous malformation(AVM)and venous malformation(VM).Methods The US characteristics with 84 AVM and 85 VM were retrospectively analyzed.Results Both of them are mixed echogenic containing solid and cystic components,heterogeneous,several no-echo areas of tube-like structure,reticular or honeycombed.There were statistical significances in the depth,diameter of noecho areas,vein peak(all P<0.001),and AVM was all significantly higher than VM.Meanwhile there were also statistical significances in the shape and border(all P<0.001).AVM had no calcifications,but some of VM had calcifications(16%).AVM color Doppler flow imaging(CDFI)showed that no-echo areas were full of mosaic blood flow signals.And pulse wave Doppler(PW)showed that no-echo areas of all the cases were detected arteria spectrum,most of them was high velocity and all of them was low resistant.And a great amount of cases were only detected arteria spectrum,a few of cases were detected arteria spectrum and vein spectrum,particularly in arteria spectrum.However,VM CDFI showed that no-echo areas had blood flow signals,and after compress,blood flow signals were increased,flickering phenomenon and compression test positive were visible.PW showed that no-echo areas were detected vein spectrum of low velocity.Conclusions AVM and VM demonstrate similar manifestations in ultrasound,but in CDFI and PW,they show remarkable differences.It had important value for the differential diagnosis of the two diseases.
