膀胱内翻性乳头状瘤与尿路上皮癌的超声特征比较

摘　　要：目的探讨膀胱内翻性乳头状瘤(IUP)与尿路上皮癌(UC)的超声特征。方法回顾性分析经手术和病理证实的17例IUP和51例UC患者的超声表现,并比较两者的超声特征。结果 IUP的肿瘤基底部宽度要小于UC,高宽比则更大。UC更多表现为位于膀胱侧壁、肿瘤内可见强回声、镶边征及Ⅱ级血流信号;而钟摆征在IUP中则更为常见,上述差异均有统计学意义(P<0.05)。以肿瘤的高宽比来鉴别IUP和UC的ROC曲线下面积为0.905、最佳截断值为1.026、最佳截断值所对应的灵敏度为77.97%、特异度为97.12%。结论超声有助于IUP和UC的鉴别诊断。Objective To investigate the sonographic features of inverted urothelial papilloma(IUP)and urothelial carcinoma(UC)of the bladder.Methods Ultrasonic appearances of 17 patients with IUP and 51 patients with UC confirmed by surgery and pathology were analyzed retrospectively and compared.Results IUP has a smaller base width and a greater ratio of height to width when compared with UC.In contrast,UC often occurs in the lateral wall of bladder,with hyperechogenicity,fimbriated sign and blood flow signal of levelⅡ within tumor.Besides,pendulum signs are more likely observed in IUP.There was statistically significant difference in the aspects above(P<0.05).The area under the ROC curve was 0.905 for the differentiation between IUP and UC by using the ratio of height to width,with the cut-off value being 1.026 corresponding to the sensitivity of 77.97%and specificity of 97.12%.Conclusions Ultrasonography is helpful to the differential diagnosis of IUP and UC.
