彩色多普勒超声在2型糖尿病合并亚临床甲减患者下肢动脉硬化诊断价值

摘　　要：目的应用超声检测2型糖尿病合并亚临床甲减患者下肢动脉硬化,评价其诊断价值。方法选取患者90例,其中45例2型糖尿病合并亚临床甲减患者作为研究组,45例2型糖尿病患者作为对照组,对比两组间临床、生化指标、下肢动脉超声检查结果。结果研究组动脉内-中膜较对照组增厚,差异无统计学意义(P>0.05),与对照组比较,研究组TC、TG、LDL-C、TSH、TPOAb、TGAb水平升高,下肢动脉血管阻力指数、膝以下动脉斑块发生率、狭窄发生率、峰值流速均高于对照组,差异有统计学意义(P<0.05)。结论彩超可评估下肢动脉粥样硬化病变,协助临床干预治疗亚临床甲减患者,减缓动脉硬化进程。Objective Ultrasound was used to detect arteriosclerosis of lower extremity in patients with type 2 diabetes mellitus complicated with subclinical hypothyroidism and evaluate its diagnostic value.Methods 90 patients were selected,45 patients with type 2 diabetes mellitus complicated with subclinical hypothyroidism as the study group,45 patients with type 2 diabetes mellitus as the control group,and the clinical,biochemical indicators,lower extremity arterial ultrasound results were compared between the two groups.Results The intima-media thickness in the study group was significantly thicker than that in the control group(P>0.05).Compared with the control group,the levels of TC,TG,LDL-C,TSH,TPOAb and TGAb in the study group were higher,and the lower extremity arterial resistance index,the incidence of subgenicular artery plaque,the incidence of stenosis and peak velocity were higher than those in the control group.There was statistical significance(P<0.05).Conclusions Color Doppler ultrasound can evaluate atherosclerotic lesions of lower extremities,assist clinical intervention in the treatment of subclinical hypothyroidism,and slow down the process of atherosclerosis.
