三种超声检查方法对RA患者膝关节滑膜血管翳检查结果的比较

摘　　要：目的比较CDFI、PDUS与SMI对RA患者膝关节滑膜血流的检出效果,探讨SMI在膝关节滑膜血管翳的应用价值。方法分别用CDFI、PDUS及SMI对49个预行全膝关节置换术的膝关节髌上囊滑膜血流信号进行检测、记录并进行分级,对三种超声检查方法的检测结果进行对比分析。并将SMI分级与滑膜病理分级进行比较。结果 41例患者,行全膝关节置换术的膝关节49个。CDFI模式滑膜血流信号显示率为93.9%,PDUS模式滑膜血流信号显示率为97.9%,SMI模式滑膜血流信号显示率为100%,三种超声检查方法结果比较差异有统计学意义(HC=11.84,P<0.05)。SMI分级与滑膜病理分级的一致性较好(Kappa=0.639,P<0.05)。结论与CDFI和PDUS相比,SMI对RA患者滑膜血流信号的显示率更高,SMI分级与病理分级的一致性较好,SMI对评估RA疾病的活动度有一定的应用价值。Objective To compare the detection effects of CDFI,PDUS and SMI on synovial blood flow in RA patients and to explore the application value of SMI in synovial vasospasm of knee joints.Methods The blood flow signals of the supraorbital bursa ofthe knee joint of 49 preoperative total knee arthroplasties were measured,recorded and graded by CDFI,PDUS,and SMI,respectively.The results of the three ultrasound examination methods were compared and analyzed.The SMI grading was compared with the pathological grade of the synovial membrane.Results Forty-one patients had 49 total knee arthroplasties.The display rate of synovial blood flow signal was 93.9% in CDFI model,97.9% in PDUS model synovial blood flow signal,and 100%in SMI model synovial blood flow signal.There were statistically significant differences in the results of the three ultrasound examination methods.Significance(HC=11.84,P<0.05).The consistency of SMI grade and pathological gradeof synovial membrane was better(Kappa=0.639,P<0.05).Conclusions Compared with CDFI and PDUS,SMI showed higher rate of synovial blood flow signal in RA patients,SMI classification had better consistency with pathological grade,SMI had certain application value in assessing the activity of RA disease.
