胃祺饮加减治疗气虚血瘀型慢性萎缩性胃炎32例

摘　　要：目的观察胃祺饮加减治疗气虚血瘀型慢性萎缩性胃炎的临床及病理疗效。方法将62例气虚血瘀型慢性萎缩性胃炎患者随机分为治疗组(32例)和对照组(30例)。两组均给予西医常规治疗,对照组在此基础上加用硒酵母胶囊及替普瑞酮胶囊,治疗组予胃祺饮加减治疗。两组均治疗3个月,观察临床疗效、病理疗效,比较中医证候积分的变化情况。结果(1)治疗组、对照组总有效率分别为90.6%、56.7%;两组临床疗效比较,治疗组优于对照组(P<0.05)。(2)治疗前后组内比较,两组中医证候积分均较治疗前减少(P<0.05);组间治疗后中医证候积分差值比较,差异有统计学意义(P<0.05)。(3)治疗组、对照组腺体萎缩的总有效率分别为62.5%、30.0%,肠上皮化生的总有效率分别为58.1%、27.6%;两组病理疗效比较,治疗组优于对照组(P<0.05)。结论胃祺饮加减治疗气虚血瘀型慢性萎缩性胃炎疗效满意,能明显改善患者的临床症状,并在一定程度上逆转胃黏膜固有腺体的萎缩及肠上皮化生。Objective To observe the clinical and pathological effects of Weiqi Decoction on the treatment of chronic atrophic gastritis with qi deficiency and blood stasis. Methods Sixty-two patients with chronic atrophic gastritis with qi deficiency and blood stasis were randomly divided into treatment group( 32 cases) and control group( 30 cases). Both groups were given conventional treatment of Western medicine,but selenium yeast capsules and teprenone capsules were added into the control group on this basis and the treatment group was added with Weiqi Decoction. Both groups were treated for3 months to observe the clinical efficacy and pathological changes,and to compare the changes of TCM syndrome scores. Results(1) The total effective rate of the treatment group and the control group were 90.6% and 56.7% respectively,and the clinical efficacy of the treatment group was better than that of the control group( P<0.05).(2)After treatment,the scores of TCM syndromes in the two groups were decreased compared with those before treatment( P<0.05),and the difference of TCM syndromes between the two groups after treatment was statistically significant( P<0.05).(3) The total effective rate of gland atrophy in the treatment group and the control group was 62.5% and 30.0%,and the total effective rate of intestinal metaplasia was 58.1%and 27. 6%,respectively. The improvement of pathological changes in the treatment group was better than that in the control group( P<0.05).Conclusion The effect of Weiqi Decoction on treating chronic atrophic gastritis with deficiency of qi and blood stasis is satisfactory. It can obviously improve the clinical symptoms of the patients and to some extent reverse the atrophy of the inherent glands of gastric mucosa and intestinal metaplasia.
