摘 要:目的探讨补钠对颅脑损伤后低钠血症患者的临床疗效。方法收集84例颅脑损伤后低钠血症患者为研究对象,将所选的患者分为两组。对照组患者进行常规的治疗;观察组患者通过补充高浓度的氯化钠进行治疗。然后比较两组患者血钠的纠正率、血钠恢复正常水平的时间、抗利尿激素分泌失调综合征(SIADH)的发生率、死亡率、满意度及患者血钠恢复后的复发率。结果患者血钠水平恢复的时间观察组明显短于对照组;满意度及治疗第5天后低血钠得到纠正的患者观察组明显均高于对照组;观察组患者血清钠纠正后的复发率及病死率均明显低于对照组;治疗过程中观察组患者SIADH的发生率观察组低于对照组,但两组患者间差异无统计学意义(P> 0.05)。结论高浓度的氯化钠能快速恢复颅脑损伤后低钠血症患者血清中钠离子的正常水平,提高患者血钠的纠正率,降低患者血清钠纠正后的复发率及病死率,值得在临床治疗过程中大力推广应用。Objective To investigate the clinical effect of sodium supplementation on hyponatremia patients after craniocerebral injury. Methods Eighty-four patients with hyponatremia after craniocerebral injury were collected as the research objects and divided into two groups.The control group was given routine treatment,while the observation group was supplemented with high concentration of sodium chloride.The corrective rate of serum sodium,the recovery time of blood sodium level,the incidence of SIADH,mortality,satisfaction and the recurrence rate of serum sodium after recovery were compared between the two groups. Results The recovery time of blood sodium level in the observation group was significantly shorter than that in the control group.The satisfaction and the proportion of patients with hyponatremia corrected after the 5 th day of treatment in the observation group was significantly higher than those in the control group.The recurrence rate and mortality in the observation group after serum sodium correction were significantly lower than those in the control group.In the course of treatment,the incidence of SIADH in the observation group was lower than that in the control group,but there was no significant difference between the two groups(P >0.05).Conclusion High concentration of sodium chloride can quickly restore the normal level of serum sodium in patients with hyponatremia after craniocerebral injury,improve the correction rate of serum sodium,reduce the recurrence rate and mortality of patients after serum sodium correction,which is worthy of vigorous promotion and application in clinical treatment.
