“双尾翼”锁扣带袢钛板重建喙锁韧带治疗新鲜肩锁关节脱位的疗效

摘　　要：目的观察应用"双尾翼"锁扣带袢钛板重建喙锁韧带治疗新鲜RockwoodⅢ或Ⅳ型肩锁关节脱位的临床效果。方法选取新鲜RockwoodⅢ或Ⅳ型肩锁关节脱位患者15例,手术距受伤时间不超过2周,应用"双尾翼"锁扣带袢钛板重建喙锁韧带治疗。随访12个月,观察手术效果,行肩关节功能Constant评分。结果 15例患者手术顺利。术前肩关节Constant评分为(62.02±1.26)分,术后6个月改善为(88.23±1.69)分,肩关节活动范围较术前明显增加。术后未发生切口感染、慢性疼痛、内固定松动、断裂、再发脱位和继发骨折等并发症。结论采用"双尾翼"锁扣带袢钛板重建喙锁韧带治疗新鲜肩锁关节脱位,手术创伤小,操作简单,术后早期可进行肩关节功能锻炼,临床效果较好。Objective To observe the clinical efficacy of endobutton plate internal fixation for the treatment of fresh Rockwood Ⅲ-Ⅳ acromioclavicular joint dislocation.Methods The "doubletailed"locking band loop titanium plate was used to reconstruct the coracoclavicular ligament in 15 patients with Rockwood Ⅲ-Ⅳ acromioclavicular joint dislocation,who had the time of less than 2 weeks after injury.The patients were followed up for 12 months.The clinical efficacy was observed and shoulder function was evaluated with Constant scoring.Results The operation was successful in all cases.Constant score was(62.02±1.26)points before surgery,which was(88.23±1.69)points 6 months after operation with an increased range of shoulder movement.No obvious complications occurred after operation.Conclusion To reconstruct the coracoclavicular ligament with endobutton plate is an effective method for the treatment of fresh acromioclavicular joint dislocation.
