摘　　要：目的探讨新生儿自发性胃破裂床旁手术围术期的护理方法,旨在提高临床护理质量。方法回顾性分析中国人民解放军总医院第七医学中心2011年1月至2017年12月在新生儿重症监护病房床旁完成的7例新生儿自发性胃破裂修补术的基本资料、围术期护理方法,并结合7例患儿的临床转归,总结护理经验。结果 7例患儿手术过程顺利,无术中死亡、大出血等并发症,其中2例患儿因合并新生儿重度窒息,家属担心预后差,放弃治疗,其余5例患儿术后恢复良好,痊愈出院,平均住院时间35d。结论对于自发性胃破裂的患儿,术前应积极改善呼吸及中毒性休克症状,并用床旁手术以缩短手术时间,术后严密监测生命体征、抗感染、营养支持、预防并发症等全面优质的围术期护理是手术成功的重要保证。Objective To explore the perioperative nursing of neonatal spontaneous gastric rupture,the aim is to improve the quality of care.Method Retrospective analysis of our hospital from January 2011 to December 2017 in case of neonatal NICU bed complete spontaneousgastric rupture repair basic information,perioperative nursing care.combined with the clinical outcome of 7 children,the nursingexperience was summarized.Result 7 cases of children with smooth operation,no intraoperative death and major hemorrhage,including 2 patients with severe asphyxiation of the newborn,the family worried about poor prognosis and gave up treatment.The remaining 5 childrenrecovered well after surgery and were discharged from hospital for an average of 35 days.Conclusion For children with spontaneous gastricrupture,it is necessary to improve the breathing,improve the symptoms of toxic shock and shorten the operation time.It is a guarantee forthe success of the operation to closely monitor the vital signs,anti-infection,nutritional support and prevention of complications.
