摘　　要：目的观察右美托咪定在高血压性脑出血术后应用的效果。方法对医院收治的高血压性脑出血患者94例进行回顾性分析,根据术后治疗方法差异性分为观察组和对照组,每组47例。2组均接受血肿清除术治疗,对照组术后进行常规治疗,观察组在此基础上给予右美托咪定治疗,比较2组术后不良反应(呼吸抑制、呛咳、躁动)发生率;比较2组术后24 h呼吸频率、心率以及平均动脉压水平;比较2组术后再出血发生率。结果观察组术后不良反应发生率为6. 38%,低于对照组的21. 28%(P <0. 05);观察组术后24 h呼吸频率、心率以及平均动脉压水平均低于对照组(P <0. 01或P <0. 05); 2组术后再出血发生率比较差异无统计学意义(P> 0. 05)。结论高血压性脑出血患者术后给予右美托咪定可稳定患者呼吸频率、心率以及动脉压,并降低不良反应发生率,可在一定程度上改善术后再出血。Objective To observe the clinical effect of dexmedetomidine in the treatment of hypertensive intracerebral hemorrhage after operation. Methods The clinical data of 94 patients with hypertensive cerebral hemorrhage were retrospectively analyzed. According to the different treatment methods after operation,the patients were divided into observation group and control group. Both groups were treated with hematoma clearance. The control group was treated with routine treatment after operation. The observation group was treated with dexmedetomidine on the basis of the control group. The incidence of adverse reactions( including respiratory depression,cough and restlessness),respiratory rate,heart rate and mean arterial pressure within 24 hours after operation,and the incidence of postoperative rebleeding were compared between the two groups. Results The incidence of adverse reactions in the observation group was 6. 38%,lower than 21. 28% in the control group( P <0. 05);the 24-hour respiratory rate,heart rate and mean arterial pressure in the observation group were lower than those in the control group( P < 0. 01 or P < 0. 05);there was no significant difference in the incidence of re-bleeding between the two groups( P > 0. 05). Conclusion The patients with hypertensive intracerebral hemorrhage were treated with dexmedetomidine after operation,which could stabilize the respiratory rate,heart rate and arterial pressure. At the same time,it can reduce the risk of adverse reactions and control the incidence of postoperative rebleeding to a certain extent.
