摘 要:目的:对目前国内外发布的儿童哮喘相关临床实践指南进行质量评价及内容分析,为规范化构建我国儿童哮喘健康教育方案提供借鉴和参考依据。方法:系统检索近5年国内外临床实践指南网站、哮喘专业协会网站及各数据库发布的儿童哮喘相关指南,采用AGREEⅡ工具对符合纳入、排除标准的指南进行质量评价。结果:共检索出符合要求的指南9部,8部来自国外,1部来自国内,其中6部为循证指南。AGREEⅡ各领域标准化得分:范围和目的为52.78%、参与人员为37.04%、制定的严谨性为27.89%、清晰性为47.84%、应用性为31.25%及编辑的独立性为24.54%。指南总体质量评价为5部B级,4部C级。通过内容分析,形成儿童哮喘教育与管理推荐意见35条。结论:目前现有的9部儿童哮喘相关临床实践指南总体质量一般,指南开发的严谨性、清晰性、应用性及编辑的独立性有待提高。质量较好的指南对我国儿童哮喘管理的临床实践有一定指导意义,可借鉴国外高质量证据进行本土化实践,指导我国儿童哮喘教育与管理领域的发展。Objective:To evaluate the quality and analyze the contents of the clinical practice guidelines on childhood asthma,and to provide references for the construction of health education program for children with asthma in China.Methods:Guidelines on childhood asthma from websites of clinical practice guidelines,asthma professional organizations and databases published internationally in recent 5 years were searched,then the quality evaluation of included guidelines was carded out using AGREE II.Results:We identified 9 guidelines,of which 8 were in English,one was in Chinese,and 6 were evidence-based guidelines.The average AGREE Ⅱ standardized scores in 6 domains were 52.78%(scope and purpose),37.04%(stakeholder involvement),27.89%(rigor of development),47.84%(clarity of presentations),31.25%(applicability) and 24.54%(independence),respectively.For overall quality,5 of them were grade B and 4 of them were grade C.Totally 35 recommendations of education and management on childhood asthma were extracted.Conclusion:The qualities of included guidelines were at the average level.The rigor of development,clarity of presentations,applicability and independence of guidelines need to be improved.We should learn from foreign high-quality evidence for localization practice,and to guide the development of asthma education and management of children in China.
