摘 要:目的从代谢组学的层面探讨亚低温对心脏骤停的脑保护作用,为临床实现最佳亚低温治疗温度和时间的选择提供依据和参考。方法采用窒息法制作心脏骤停模型,将18只自主循环恢复的家兔随机分为常温治疗组和亚低温组治疗组,6只单纯气管切开的家兔作为假手术组。GC/MS平台检测Oh,自主循环恢复后30min、3h、6h、24h后血浆代谢组学的改变,采用Omiesbean组学分析平台进行代谢组学的生物信息学分析。结果成果制作了窒息所致的心脏骤停模型,亚低温改善心脏骤停家兔的脑损伤,增加心脏骤停的存活率,血浆代谢产物及其代谢轮廓发生明显改变。结论亚低温治疗有助于代谢谱向正常转变,促进大脑功能的恢复,改善心脏骤停所致的存活率。Objective To clarify the brain protective effect of Hypothermia on cardiac arrest Using Metabonomics technology,and contribute to provide the basis and reference for the optimum temperature and time of mild hypothermia treatment in clinc.Methods Rabbit models of cardiac arrest was established by asphyxia.After the restoration of spontaneous circulation (ROSC),18 rabbits were randomly divided into normothermia and mild hypotherrnia therapy groups,and another 6 rabbits were divided into sham group which were subjected to surgery without induction of ventricular fibrillation by asphyxia.The following five time points were selected:before cardiac arrest,30min after ROSC (the mild hypothermia therapy group reached the target temperature),3h after ROSC (the mild hypothermia therapy group before warming), 6h after ROSC (the mild hypothermia therapy group recovered to normal temperature),and 24h after ROSC.The serum metabolites of all time points in three different groups (the sham group,norrnothermia group and hypothermia therapy group)were detected by GC/MC.The Omiesbean,a genomics-proteomicmetabonomics analysis platform,was used to analyze the bioinformatics of metabonomics.Results Asphyxiation-induced cardiac arrest in rabbits was produced successfully,Mild hypothermia therapy could protect the cerebral function from cardiac arrest-induced damage in rabbits,and increased the survival ratio of rabbits undergoing the cardiac arrest.The plasma metabonomic profile was changed obviously.Conclusions Mild hypothermia therapy can reduce the heart rate,contribute the metabolic profiles transition to normal, promote the recovery of cerebral function induced by cardiac arrest in rabbits,as well as improve the survival rates.
