摘 要:目的探讨磁共振成像(MRI)技术在胎盘植入诊断与深度判定中的应用价值。方法选取我院2012年3月—2018年3月收治的疑似胎盘植入患者65例,对其进行产前MRI检查,对比手术及病理诊断结果,分析其灵敏度、特异度、阳性预测值、阴性预测值。结果 MRI可显示胎盘底蜕膜、子宫肌层及浆膜层的结构,其MRI征象有胎盘-子宫肌层界限模糊、子宫肌层变薄或中断、SSh-FSE显示低信号带,B-FFE显示高信号带,以及信号不均、膀胱壁不完整、局部胎盘膨隆。MRI确诊胎盘植入50例,MRI检查阳性48例,阴性17例,其中有3例误诊,5例漏诊。其灵敏度、特异度、阳性预测值、阴性预测值分别为90.00%、80.00%、93.75%、70.59%。结论 MRI诊断胎盘植入灵敏度高,可较准确的判定植入深度和范围,对临床后续治疗计划方针具有指导性作用。Objective To explore the value of magnetic resonance imaging( MRI) in the diagnosis and depth determination of placenta implantation.Methods From March 2012 to March 2018,sixty-five patients with suspected placenta implantation were enrolled in our hospital.Prenatal MRI was performed.Compared the surgical and pathological diagnose results.The sensitivity and specificity,positive predictive value,negative predictive value of MRI were analyzed.Results MRI could reveal decidua of placenta floor, and the structure of myometrium and serosal layer.The MRI signs including blurred in the placenta-uterine myometrium,thinning or interruption of the myometrium,low signal band in the SSH-FSE,B-FFE showed a high signal band,as well as uneven signal,incomplete bladder wall,and partial placental bulging.MRI confirmed 50 cases of placenta implantation,48 cases were positive according to MRI,and 17 cases were negative,of which 3 cases were misdiagnosed and 5 cases were missed.The sensitivity,specificity,positive predictive value,and negative predictive value were 90.00%,80.00%,93.75%,and 70.59%,respectively.Conclusions MRI diagnosis performs high sensitivity of placenta implantation, it could accurately determine the depth and scope of implantation,and has a guiding role in follow-up clinical treatment planning.
