重度子宫内膜异位症腹腔镜术后采用GnRHa联合反向添加戊酸雌二醇治疗的效果分析

摘　　要：目的:探讨促性腺激素释放激素激动剂(GnRHa)联合反向添加戊酸雌二醇治疗腹腔镜术后重度子宫内膜异位症(s EMT)患者的临床效果。方法:90例s EMT患者随机分为对照组:仅做腹腔镜手术; GnRHa组:腹腔镜手术后用GnRHa治疗;反向添加组:腹腔镜手术后用GnRHa联合反向添加戊酸雌二醇治疗,比较各组间的治疗效果。结果:治疗后,GnRHa组、反向添加组的VAS评分、复发率均明显低于对照组,差异有统计学意义(P<0.05); GnRHa组与反向添加组的VAS评分、复发率差异无统计学意义(P>0.05),但反向添加组的不良反应率明显低于GnRHa组,差异有统计学意义(P<0.05)。结论:反向添加戊酸雌二醇可有效降低GnRHa治疗s EMT腹腔镜术后患者的不良反应。Objective:To investigate the clinical effect of gonadotropin releasing hormone agonist(GnRHa)combined with add-back estradiol valerate in the treatment of severe endometriosis(s EMT) after laparoscopic surgery.Methods:90 patients with s EMT were randomly divided into control group:only laparoscopic surgery;GnRHa group:treated with GnRHa after laparoscopic surgery;add-back group:treated with GnRHa combined with add-back group estradiol valerate after laparoscopic surgery,and compared the therapeutic effects among the groups.Results:After treatment,the VAS score and recurrence rate of GnRHa group and add-back group were significantly lower than those of control group(P<0.05);there was no significant difference in VAS score and recurrence rate between GnRHa group and add-back group(P>0.05),but the adverse reaction rate of add-back group was significantly lower than that of GnRHa group,the difference was statistically significant(P<0.05).Conclusion:Add-back estradiol valerate can effectively reduce the adverse reactions of GnRHa in the treatment of s EMT patients after laparoscopic surgery.
