重度慢性心力衰竭患者姑息照护需求水平及影响因素分析

摘　　要：目的 探讨重度慢性心力衰竭(CHF)患者姑息照护需求水平及其影响因素。 方法 采用便利抽样方法,选择2016年6月—2017年10月在哈尔滨市某三甲医院心血管内科住院的110例重度CHF患者为研究对象。采用一般资料调查问卷、姑息照护结局量表(POS)、医院焦虑抑郁量表、卡氏功能状态量表(KPS)对其进行调查。采用多元线性回归分析对重度CHF患者姑息照护需求的影响因素进行分析。 结果 110例重度CHF患者POS得分(19.01±4.20)分,多元线性回归分析显示,患者病程时间、心功能、合并症个数、焦虑、抑郁水平是重度CHF患者姑息照护需求水平的影响因素(P<0.05)。 结论 重度CHF患者对姑息照护需求高,医护人员应根据患者实际情况,尽早制定并实施姑息照护措施,进一步改善患者的生活质量。Objective To explore the palliative care needs in patients with severe and chronic heart failure (CHF) and their influencing factors.Methods Totally 110 severe CHF patients hospitalized in the Department of Cardiovascular Medicine of a ClassⅢ Grade A hospital in Harbin from June 2016 to October 2017 were selected by convenient sampling, and investigated with the general information questionnaire, Palliative Care Outcome Scale (POS) , Hospital Anxiety and Depression Scale (HADS) and Karnofsky Performance Status (KPS) . Multiple linear regression analysis was used to analyze the influencing factors to the palliative care needs in severe CHF patients.Results The POS score of the 110 severe CHF patients was (19.01±4.20) . Multiple linear regression analysis showed that the patient's course of disease, cardiac function, number of comorbidities, anxiety and depression were the influencing factors to the palliative care needs in severe CHF patients (P<0.05) .Conclusions Severe CHF patients have higher palliative care needs. Medical and nursing workers should take palliative care measures as early as possible according to the actual situation of patients to improve the patient's quality of life.
