全血γ-干扰素释放试验在风湿病并结核病中的诊断价值

摘　　要：目的探讨全血γ-干扰素释放试验(IGRA)在风湿病并结核病中的诊断价值。方法选取梧州市红十字会医院2015年1月—2017年1月收治的风湿病患者60例,均给予IGRA、结核菌素皮肤试验(TST)进行检测。以抗酸杆菌涂片及抗酸杆菌培养结果为金标准,比较IGRA和TST两种方法检测的灵敏度、特异度及准确率。结果经抗酸杆菌涂片及抗酸杆菌培养检查确诊风湿病并结核病患者47例,IGRA检测的灵敏度为93.6%(44/47),特异度为84.6%(11/13),准确率为95.8%(44/46),而TST检测分别为57.4%(27/47)、46.2%(6/13)、79.4%(27/34)。结论 IGRA检测诊断风湿病并结核病不仅具有较高的敏感度及特异度,且方便快捷、操作简单。Objective To discuss the diagnostic value of whole blood γ-interferon release test(IGRA) on rheumatic disease complicated with tuberculosis.Methods A total of 60 cases of rheumatic disease patients were selected from January 2015 to January 2017 in Wuzhou Red Cross Hospital,all patients were tested with IGRA and tuberculin skin test(TST).Taken acid-fast bacilli smears and acid-fast bacilli culture results as gold standard,the sensitivity,specificity and accuracy rate were compared between the results of IGRA and TST.Results The rheumatic disease complicated with tuberculosis was made by acid-fast bacilli smears and acid-fast bacilli culture tests was 47 cases,the sensitivity,specificity and accuracy of IGRA respectively were 93.6%(44/47),84.6%(11/13),95.8%(44/46),and TST respectively were 57.4%(27/47),46.2%(6/13),79.4%(27/34).Conclusion IGRA not only has higher sensitivity and specificity in the diagnose of rheumatic disease complicated with tuberculosis,and it is convenient,quick and simple to operate.
