摘  要:目的评价磁共振成像弥散加权及动态增强对乳腺增生性疾病的诊断价值。方法回顾性分析62例经手术或活检证实的乳腺增生性疾病MRI弥散加权和动态增强特征。结果乳腺增生性疾病在动态增强多表现为非肿块样强化(40/62,65%),动态增强时间信号曲线(DCE—TIC)多为I型(22/62,35%)和Ⅱ型(25/62,40%),弥散加权(DWI)表现为等或稍高信号,b值在1 000时,表观弥散系数(ADC)值>1.2×10^3mm^2/s。结论乳腺增生性疾病MRI表现多样,不同的阶段有不同的表现,DCE-TIC以上升及平台为主,Ⅰ-Ⅲ型乳腺增生性疾病弥漫受限不明显,而Ⅳ型(囊性增生型)可见弥散受限,囊肿随着b值的增大信号强化明显衰减,有助于正确诊断。Objective To evaluate the diagnostic value of magnetic resonance imaging (MRI)diffusion-weighted and dynamic enhancement in breast proliferative diseases.Methods MRI,diffusion weighted and dynamic enhancement features were analyzed retrospectively in 62 cases of breast proliferative diseases confirmed by surgery or biopsy.Results Dynamic enhancement of breast proliferative diseases was usually non-tumor-like enhancement (40/62,65%), and the dynamic contrast enhancement time intensity curve (DCE -TIC)was mostly typeⅠ(22/62,35%)and Ⅱ (25/62,40%).Diffusion weighted imaging (DWI)presented equal or slightly higher signal.Apparent diffusion coefficients (ADC)value was >1.2×10^-3 mm^2/s for b value of 1000.Conclusions The manifestations of breast prolif- erative diseases on MRI are diverse,and different stages have different manifestations.The time signal curve is mainly ascending and platform,and diffusion limitation of type Ⅰ-Ⅲ breast hyperplasia is not obvious,but type Ⅳ (cystic hyperplasia)is visible.The signal intensity of cyst decreases obviously with the increase of b value,which is helpful for correct diagnosis.
