摘 要:目的系统评价早期康复路径对急性脑梗塞患者功能恢复的影响,为临床护理提供参考依据。方法检索中外文献数据库(检索CNKI、VIP、Wang Fang及Pub Med、the Science Dicrect、Cochrane Library数据库),收集与本研究有关的随机对照试验,文献质量评价后,用Stata12.0软件进行Meta分析。结果共纳入相关文献15篇,共1 307例患者(对照组639例,试验组668例),结果显示:与对照组相比,干预组患者运动功能明显得到改善[SMD=0.86, 95%CI(0.70, 1.02),P<0.001],神经功能缺损程度明显降低[SMD=-0.85, 95%CI(-1.21,-0.49), P<0.001],神经功能康复程度得到有效提高[RR=1.24, 95%CI(1.12, 1.37), P<0.001],2组间差异均具有统计学意义。结论当前证据显示对急性脑梗塞患者实施早期康复训练能促进患者运动功能恢复,改善神经功能缺损程度,提高生活质量。Objective The aim of this study was to systematically evaluate the effect of early rehabilitation pathway on functional recovery of patients with acute cerebral infarction and provide reference for clinical nursing. Methods Chinese and foreign literature databases were searched(CNKI, VIP, WangFang, PubMed, the Science Dicrect, and Cochrane Library databases). Randomized controlled trials related to this study were collected. After quality evaluation of the literatures, metaanalysis was performed using Stata12.0 software. Results A total of 15 related articles were included with 1307 patients(639 in the control group and 668 in the experimental group). The results showed that the motor function of the intervention group was significantly improved compared with the control group [SMD=0.86, 95%CI(0.70, 1.02), P<0.001]. The degree of neurologic impairment was significantly reduced [SMD =-0.85, 95% CI(-1.21,-0.49), P <0.001], and the degree of neurological function recovery was effectively improved [RR =1.24, 95% CI(1.12, 1.37), P <0.001]. The difference between the two groups were statistically significant. Conclusion Current evidence showed that early rehabilitation training for patients with acute cerebral infarction could promote recovery of motor function, improve neurological deficits, and improve quality of life.
