芪苈强心胶囊对老年重度心力衰竭患者的疗效观察

摘　　要：目的对老年重度慢性心力衰竭患者给予芪历强心胶囊治疗,并观察其临床治疗效果。方法选取2016年10月-2017年1月在第二军医大学附属长海医院50例老年慢性心力衰竭(chronic heartfailure,CHF)患者,按人院先后随机分为治疗组和对照组,各25例,对照组给予常规西药治疗,治疗组在对照组基础上加用芪苈强心胶囊,连续治疗90 d,对比2组治疗前后左室射血分数(1eft ventricular ejection fraction,LVEF)、左室舒张末期内径(1eft ventricular end diastolic diameter,LVEDD),血浆N末端脑钠肽前体(N terminal pro B type natriuretie peptide,NT-ProBNP)及临床疗效。结果治疗90d后,两组患者的血浆NT-proBNP均发生明显降低,治疗组相比于对照组降幅更明显(P=0.0327),NT-proBNP降低至少30%的患者比例为60%,对照组为44%(P=0.0094)。此外,治疗组在左心室射血分数、左心室舒张末期内径等方面的改善优于对照组(P<0.05)。结论芪苈强心胶囊联合常规西药治疗能增强老年极重度心衰患者心功能,有助于改善预后。Objective To observe the clinical effects of Qili Qiangxin capsules in elder patients with extremely severe chronic heart failure.Methods Fifty eider patients with extremely severe chronic heart failure were randomly assigned to 2 equal groups:control group receiving conventional therapy,and Qili Qiangxin capsule group receiving Qili Qiangxin capsules in addition.90 days later,the change in plasma N terminal pro B type natriuretic peptide (NT-proBNP)level,left ventricular ejection fraction (LVEF),left ventricular end diastolic diameter (LVDD)were compared.Results The NT-proBNP levels of these 2 group both decreased compared with their baseline values (both P<0.05),however,the decrease range of the Qili Qiangxin capsule group was significantly higher than that of the control group (P=0.0327).60% of the patients in the Qili Qiangxin capsule group showed a decrease range of NT-proBNP ≥30%,a percentage significantly higher than that of the control group (P=0.0094).There was no significant difference in the major measurement indicators of echocardiography before treatment.The LVEF level increased after treatment in both groups,however,the increase range of the Qili Qiangxin capsule group was significantly higher than that of the control group (P=0.0483),and the LVD of both groups decreased significantly (both P<0.05),however,the decrease range of the Qili Qiangxin capsule was significantly higher than that of the control group (P=0.0451).Conclusion In addition to conventional medications ,Qili Qiangxin capsules helps improve the heart function and prognosis of elder patients with extremely severe chronic heart failure.
