摘 要:【目的】 探讨慢性阻塞性肺疾病(COPD)患者诱导痰液和血清中可溶性晚期糖基化终末产物受体(sRAGE)和白细胞介素-32(IL-32)水平及其与肺功能的关系。【方法】 选取2017年9月至2018年10月本院收治的COPD患者89例作为研究对象(观察组),其中急性加重期患者33例,稳定期患者56例,同期选取健康志愿者90例作为对照组。检测患者诱导痰和血清中sRAGE和IL-32水平,同时检测患者肺功能指标。【结果】 观察组诱导痰及血清中sRAGE水平明显低于对照组、IL-32水平明显高于对照组(P<0.05)。COPD急性加重期组诱导痰及血清sRAGE水平明显低于稳定期组(P<0.05),而IL-32水平明显高于稳定期组(P<0.05);COPD急性加重期第一秒用力呼气容积(FEV1)/用力肺活量(FVC)、FEV1占预计值百分比明显低于稳定期组(P<0.05)。诱导痰sRAGE和IL-32与血清sRAGE和IL-32呈正相关(r=0.463,P<0.05,r=0.480,P<0.05);诱导痰sRAGE或IL-32与FEV1/FVC、FEV1占预计值百分比呈负相关(r=-4.332、-4.402,P<0.05或r=-450、-4.462,P<0.05)。【结论】 COPD患者诱导痰以及血清中sRAGE降低,而IL-32升高,其水平与肝功能密切相关,诱导痰sRAGE和IL-32水平可反映患者肺功能状况。【Objective】 To investigate the levels and significance of soluble advanced glycation end product receptor (sRAGE) and Interleukin-32 (IL-32) in induced sputum and serum of patients with chronic obstructive pulmonary disease (COPD). 【Methods】 Eighty-nine COPD patients (COPD group) treated in our hospital from September 2017 to October 2018 were selected, including 33 acute exacerbation patients and 56 stable patients, 90 healthy volunteers were selected as control group, the induced sputum and serum levels of sRAGE and IL-32 were measured. At the same time, the pulmonary function index was detected.【Results】 The levels of sRAGE and IL-32 in induced sputum and serum in the observation group were significantly lower than those in the control group (P<0.05). The levels of induced sputum and serum sRAGE in the acute exacerbation group were significantly lower than those in the stable phase group (P<0.05). The level of IL-32 was significantly higher than that of the stable group (P<0.05). The percentage of forced expiratory volume (FEV1) / forced vital capacity (FVC)and FEV1 in the first second of acute exacerbation of COPD was significantly lower than that in the stable group (P<0.05). Induced sputum sRAGE and IL-32 were positively correlated with serum sRAGE and IL-32 (r=0.463, P<0.05, r=0.480, P<0.05);induced sputum sRAGE or IL-32 was negatively correlated with the percentage of FEV1/FVC and FEV1(r=-4.332, -4.402, P<0.05 or r=-450, -4.462, P<0.05).【Conclusion】 The levels of sRAGE and IL-32 in induced sputum and serum of COPD patients decreased, while IL-32 increased. The levels of sRAGE and IL-32 in induced sputum were closely related to liver function. The levels of sRAGE and IL-32 in induced sputum could reflect the pulmonary function of COPD patients.
